纤维支气管镜下吸痰配合盐酸氨溴索治疗老年重症肺炎患者的效果

摘　　要：目的观察纤维支气管镜下吸痰配合盐酸氨溴索治疗老年重症肺炎患者的临床效果。方法选取2019年2月至2020年1月本院收治的280例老年重症肺炎患者作为研究对象,根据治疗方式不同将其分为对照组和观察组,每组140例。对照组采用纤维支气管镜吸痰治疗,观察组采用纤维支气管镜吸痰配合盐酸氨溴索治疗。比较两组的临床疗效、肺功能及呼吸指标、炎症因子水平、肺部感染程度。结果观察组的治疗总有效率显著高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组FEV_1、PEF、OI均升高,WOB均下降,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组WBC、CRP、IL-8水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组CPIS评分均降低,且观察组低于对照组(P<0.05)。结论纤维支气管镜下吸痰配合盐酸氨溴索治疗老年重症肺炎患者能够明显改善患者的肺功能及呼吸指标、炎症因子水平,提高临床治疗效果,减轻患者肺部感染程度。Objective To observe the clinical effect of sputum suction under fiberoptic bronchoscope combined with ambroxol hydrochloride in the treatment of elderly patients with severe pneumonia.Methods Two hundred and eighty elderly patients with severe pneumonia admitted in our hospital from February 2019 to January 2020 were selected as the research objects.According to different treatment methods,the patients were divided into control group and observation group,with 140 cases in each group.The control group was treated with sputum suction under fiberoptic bronchoscope,and the observation group was treated with sputum suction under fiberoptic bronchoscope combined with ambroxol hydrochloride.The clinical efficacy,pulmonary function and respiratory indexes,inflammatory factors levels and pulmonary infection degree of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,FEV1,PEF and OI of the two groups increased,WOB decreased,and those of the observation group were better than the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of WBC,CRP and IL-8 in the two groups decreased,and those of the observation group were lower than the control group,and the differences were statistically significant(P<0.05).After treatment,CPIS scores of the two groups decreased,and that of the observation group was lower than the control group(P<0.05).Conclusion Sputum suction under fiberoptic bronchoscope combined with ambroxol hydrochloride in the treatment of elderly patients with severe pneumonia can significantly improve the lung function,respiratory indexes and inflammatory factors levels,enhance the clinical treatment effect,and reduce the degree of lung infection.
