摘 要:目的评价线性探针杂交技术(HAIN技术,Geno Type MTBDR plus试剂盒)对结核分枝杆菌耐药的检测效果。方法利用传统药敏试验和HAIN技术同时对邯郸市的297例培阳肺结核病人进行耐多药检测,以传统药敏试验为金标准对HAIN技术耐药检测效能进行评价。收集邯郸市临床肺结核病人痰标本培养物,对比分析传统比例法药敏试验和HAIN技术对异烟肼和利福平的耐药检测效果。结果传统比例法药敏试验检测297份培养物有295份为结核分枝杆菌,其中异烟肼(INH)耐药40例(13.4%)、利福平(RFP)耐药43例(14.5%)、耐多药肺结核18例(6.1%);线性探针技术检测297份标本中295份为结核分枝杆菌,同时检测出异烟肼(INH)耐药41例(13.8%)、利福平(RFP)耐药45例(15.1%)、耐多药肺结核18例(6.1%)。使用分析软件SPSS 13.0进行统计分析,利福平:χ^2=0.053,P=0.817,差异无统计学意义(P>0.05)。异烟肼:χ^2=0.014,P=0.905,差异无统计学意义(P>0.05)。结论HAIN耐药检测方法更灵敏、快速、准确度更高,完全适用于临床肺结核病人异烟肼、利福平的耐药检测。Objective To evaluate the efficacy of linear probe hybridization(HAIN,Geno Type MTBDR plus kit)in detecting drug resistance of mycobacterium tuberculosis.Methods Totally 297 cases of smear positive pulmonary tuberculosis in Handan City were tested for multi-drug resistance by traditional drug sensitivity test and HAIN technology.The efficacy of HAIN technology was evaluated with traditional drug sensitivity test as gold standard.The cultures of sputum samples of tuberculosis patients in Handan were collected,and the effects of traditional proportional drug sensitivity test and HAIN technique in detecting the drug resistance of isoniazid(INH)and rifampicin(RFP)were compared and analyzed.Results Totally 295 of 297 cultures were tested as mycobacterium tuberculosis by traditional proportional drug sensitivity test.Among which,40(13.4%)were resistant to INH,43(14.5%)were resistant to RFP,18(6.1%)were resistant to multidrug tuberculosis(MDR-TB).And 295 of 297 samples were tested as mycobacterium tuberculosis by linear probe technique.Among which,41 cases(13.8%)were resistant to INH,45 cases(15.1%)were resistant to RFP and 18 cases(6.1%)were resistant to MDR-TB.SPSS 13.0 was used to analyze the data,and there were no statistically significant differences in RFP(χ^2=0.053,P=0.817>0.05)and INH(χ^2=0.014,P=0.905>0.05).Conclusion HAIN drug resistance detection method is more sensitive,rapid and accurate,which is completely applicable to the clinical detection of INH and RFP resistance in patients with pulmonary tuberculosis.
