摘 要:目的探讨妊娠期糖尿病(GDM)患者血清成纤维细胞生长因子19(FGF19)浓度的变化及与瘦素(leptin)、胰岛素抵抗的关系。方法选取2017年1月至2018年12月遂宁市中心医院产科足月单胎GDM患者44例为试验组,同期足月单胎的正常妊娠期妇女44例为对照组,分别测定两组患者血FGF19、leptin、空腹血糖(FBG)、空腹胰岛素(FINS)水平,计算胰岛素抵抗指数(HOMA-IR),并分析FGF19与以上指标的相关性。结果试验组患者血清中的FGF19水平明显低于对照组,leptin、FBG、FINS、HOMA-IR明显高于对照组(P<0.05),试验组患者FGF19水平与leptin、FBG、FINS、HOMA-IR呈负相关(P<0.05),leptin、HOMA-IR是FGF19的独立危险因素(P<0.05)。结论GDM患者FGF19水平明显降低,与leptin、胰岛素抵抗呈负相关,妊娠期FGF19的减少可能参与GDM的病理生理过程。Objective To investigate the changes of serum fibroblast growth factor 19(FGF19)concentration and its relationship with leptin and insulin resistance in patients with gestational diabetes mellitus(GDM).Methods From January 2017 to December 2018,a total of 44 patients with full-term single gestation GDM in the Obstetrics Department of Suining Central Hospital were selected as the experimental group,and 44 normal pregnant women with full-term single gestation during the same period were selected as the control group.Serum FGF19,Leptin,fasting blood glucose(FBG),fasting insulin(FINS)levels were measured in the two groups.Insulin resistance index(HOMA-IR)was calculated.And the correlation between FGF19 and the above indicators was analyzed.Results Serum levels of FGF19 in the experimental group were significantly lower than those in the control group,while the levels of leptin,FBG,FINS and HOMA-IR in the experimental group were significantly higher than those in the control group(P<0.05).The level of FGF19 was negatively correlated with leptin,FBG,FINS and HOMA-IR in the experimental group(P<0.05).Leptin and HOMA-IR were independent risk factors of FGF19(P<0.05).Conclusion The level of FGF19 in GDM patients decreases significantly,which is negatively correlated with leptin and insulin resistance.The decrease of FGF19 during pregnancy may be involved in the pathophysiological process of GDM.
