2016~2018年我院镇静催眠及抗焦虑药物用药分析

摘　　要：目的探讨2016~2018年我院镇静催眠及抗焦虑药物应用情况及变化趋势,为临床用药提供依据。方法对我院2016年1月~2018年12月镇静催眠及抗焦虑药物用药金额、用药频度(DDDs)和日均费用进行回顾性分析。结果2016~2018年我院镇静催眠及抗焦虑药物销售金额呈增长趋势。丁螺环酮、坦度螺酮、劳拉西泮及奥沙西泮居用药金额前4位,5-羟色胺激动剂逐渐取代苯二氮卓类药物(BDZ)占据主导地位。3年中我院DDDs排序一直保持在前7位的是阿普唑仑、氯硝西泮、艾司唑仑、劳拉西泮、丁螺环酮、坦度螺酮及佐匹克隆。镇静催眠及抗焦虑药物B/A值为0.17~10.0,B/A比值接近1.0者占41.18%,3年来我院用药同步逐渐转好。结论我院镇静催眠及抗焦虑药物应用基本合理,5-羟色胺激动剂已成为一线镇静催眠及抗焦虑药物。我院镇静催眠及抗焦虑药物应用与用药指南存在一定出入,应重视心理行为治疗和药物治疗的综合干预。Objective To analyze the status and trends of applications for sedative-hypnotics and antianxiety drugs in our hospital from 2016 to 2018.Methods The consumption sum,defined daily doses and defined daily costs of sedative-hypnotics and antianxiety drugs in our hospital were analyzed from Jan 2016 to Dec 2018.Results The consumption sum of sedative-hypnotics and antianxiety drugs gradually increased from 2016 to 2018.The top 4 consumption sum were buspirone,tandospirone,lorazepam and oxazepam.The top 7 DDDs were alprazolam,clonazepam,estazolam,lorazepam,buspirone,tandospirone and zopiclone.The B/A value of sedative-hypnotics and antianxiety drugs ranged 0.17~10.0,and the proportion of drugs with B/A ratio close to 1.0 accounted for 41.18%,medication synchronization in our hospital gradually improved.Conclusion The application of sedative-hypnotics and antianxiety drugs in our hospital is basically reasonable.Serotonin agonist has become the first-line sedative-hypnotics and antianxiety drugs.There are some differences between the application of sedative-hypnotics and antianxiety drugs in our hospital and the medication guidelines.Attention should be paid to the comprehensive intervention of psychological behavior therapy and drug treatment.
