反复自然流产患者封闭抗体的差异性分析及对临床治疗效果的评估价值分析

摘　　要：目的:探究反复自然流产患者的封闭抗体的差异性及其在临床治疗评估中的价值。方法:以2018年2月~2019年5月在我院进行封闭抗体(BA)检测患者作为研究象,根据不同BA的检测结果进行不同组复发流产率的比较,同时对B A阴性患者进行淋巴细胞主动免疫治疗,对不同治疗疗程B A的阳性率转化率情况进行观察分析,同时对治疗后的妊娠结局进行观察分析。结果:B A阴性反复自然流产的发病率为13/31(41.93%),B A阳性反复自然流产1/11(9.09%),B A阴性的反复自然流产率要显著的高于B A阳性,具有统计学意义(P<0.05;);淋巴细胞主动免疫治疗后1和2疗程的B A阳性率均要高于治疗前,有统计学意义(P<0.05),与第1疗程相比,第2疗程的阳性率要显著升高,(P<0.05);主动免疫后B A阳性患者的再次妊娠成功率要显著高于B A阴性患者,且存在统计学差异(P<0.05)。结论:在反复流产患者中B A水平多数为阴性,同时BA在评估淋巴细胞主动免疫治疗的临床疗效上也具有一定的意义。Objective:To explore the difference of blocking antibodies in patients with recurrent spontaneous abortion and its value in clinical treatment evaluation.Methods:The patients with blocking antibody(BA)detection in our hospital from February,2018 to May,2019 were selected as the research objects.The rates of recurrent abortion in different groups were compared,according to the results of different BA tests.And the BA negative patients were treated with the active lymphocyte immunotherapy.The positive conversion rate of BA in different treatment courses were observed and analyzed,and the pregnancy outcome after treatment were also observed and analyzed.Results:The incidence of recurrent spontaneous abortion with negative BA was 13/31(41.93%).The BA positive recurrent spontaneous abortion was 1/11(9.09%),and the recurrent spontaneous abortion rate of BA negative was significantly higher than that of BA positive(P<0.05).The positive rate of BA after the active lymphocyte immunotherapy for 1 and 2 courses was higher than that before treatment,and there was statistical significance(P<0.05).Compared with the first course,the positive rate in the second course was significantly increased(P<0.05).After active immunization,the success rate of pregnancy in BA-positive patients was significantly higher than that in BA-negative patients(P<0.05).Conclusion:Most of the patients with recurrent abortion was negative BA level.BA has certain significance in evaluating the clinical effect of lymphocyte active immunotherapy.
