摘 要:目的:探讨第二代双源CT大螺距联合双流注射法在肺动脉成像中的价值。方法:选择2019年1—8月汕头市中心医院拟诊为肺动脉栓塞的67例患者,其中男性34例,女性33例,年龄23~81岁,平均(59.7±15.2)岁。采用随机数字表法分为两组,A组(34例)采用大螺距联合双流注射法方案,B组(33例)采用传统小剂量团注常规扫描方案。比较两组患者的对比剂用量、辐射剂量和图像质量差异。结果:A组对比剂用量(18.73±2.61) m L,B组对比剂用量40 mL,差异有统计学意义(P<0.001)。A组辐射剂量、上腔静脉CT值均低于B组(P<0.001),A组肺动静脉分离及上腔静脉硬束伪影评分均优于B组(P<0.001)。结论:第二代双源CT大螺距联合双流注射法可获得理想的肺动脉成像质量并大幅度降低对比剂用量和辐射剂量。Objective:To investigate the value of large pitch scan combined with dual flow injection of second generation dual source CT in pulmonary angiography.Methods:67 patients with pulmonary embolism in Shantou Central Hospital from January to August 2019 were selected,including 34 males and 33 females with an average age of(59.7±15.2)years(range,23-81 years).They were randomly divided into two groups:group A(34 cases)was treated with large pitch combined with double flow injection;group B(33 cases)was treated with conventional scanning scheme of low-dose bolus injection.The contrast agent dosage,radiation dose and image quality were compared between the two groups.Results:The dosage of contrast medium was(18.73±2.61)mL in group A and40 mL in group B(P<0.001).The radiation dose and CT value of superior vena cava in group A were lower than those in group B(P<0.001).Pulmonary arteriovenous separation and superior vena cava artifact score in group A were better than those in group B(P<0.001).Conclusion:The second generation dual source CT with high pitch and dual flow injection can obtain ideal pulmonary artery imaging quality,and greatly reduce the dosage of contrast medium and radiation dose.
