摘　　要：目的分析人类白细胞抗原-G(HLA-G)和干扰素γ(IFN-γ)与复发性流产(RPL)的相关性。方法选取2016年4月至2019年7月在本院生殖医学中心和妇产科诊治的70例流产患者为研究对象,根据流产发生的起因分组,分为RPL不孕症患者的RPL组(n=29)和正常终止妊娠的早孕女性的对照组(n=41)。采用酶联免疫反应测定两组患者血清HLA-G和IFN-γ水平,采用实时荧光定量PCR(qRT-PCR)测定两组患者外周血和流产绒毛组织中HLA-G和IFN-γ的mRNA相对表达量。结果两组患者的年龄、体质量指数(BMI)、基础卵泡刺激素(FSH)、黄体生成素(LH)和雌二醇(E2)水平等一般资料比较均无显著性差异(P>0.05),而RPL组患者在胎停7~8周的血清HCG和孕酮水平均显著低于对照组(P<0.05);酶联免疫反应结果显示,RPL组血清HLA-G水平显著低于对照组,而IFN-γ水平显著高于对照组(P<0.05);qRT-PCR结果显示,两组患者外周血和流产绒毛中均有HLA-G mRNA和IFN-γmRNA表达,RPL组患者外周血和流产绒毛中的HLA-G mRNA表达水平均显著低于对照组,而IFN-γmRNA表达均水平显著高于对照组(P<0.01)。结论妊娠初期HLA-G和IFN-γ的表达失衡与RPL的发生存在一定相关性。Objective:To explore correlation of recurrent pregnancy loss(RPL)with human leukocyte antigen-G(HLA-G)and interferon-γ(IFN-γ).Methods:Seventy abortion patients were selected in Department of Reproductive Medicine and Department of Gynecology and Obstetrics in Shanxi Provincial People’s Hospital from April 2016 to July 2019.According to the cause of abortion,they were divided into two groups:the patients with RPL infertility in RPL group(n=29)and the early-pregnant women with normal pregnancy termination in the control group(n=41).Enzyme linked immunosorbent assay was employed to detect expressions of HLA-G and IFN-γin serum of both groups,and real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)was employed to detect expression of HLA-G and IFN-γmRNA in peripheral blood and abortion tissue of both groups.Results:There were no significant differences in basal characteristics including age,body mass index(BMI),basal FSH,LH and E2 between RPL group and control group(P>0.05).The serum of HCG and progesterone in serum samples which were collected in fetal arrest at seven to eight weeks of RPL group were significantly lower compared with those of control group(P<0.05).Enzyme linked immunosorbent assay showed that the expression of HLA-G in serum of RPL group was significantly lower,and IFN-γwas significantly higher than those of control group(P<0.05).The qRT-PCR results showed that both HLA-G and IFN-γexpressed in peripheral blood and aborted tissues.The expression of HLA-G mRNA in peripheral blood leukocytes and aborted tissues of RPL group were significantly lower,but the expression of IFN-γmRNA was significantly higher compared with those expressions in control group(P<0.01).Conclusions:There is a certain correlation between the imbalance expression of HLA-G&IFN-γand RPL.
