针灸治疗颞下颌关节紊乱综合征选穴规律探析

摘　　要：目的:通过文献检索,探析针灸治疗颞下颌关节紊乱综合征(TMJDS)的选穴规律。方法:在中国知网中检索1979年1月至2018年8月发表的有关针灸治疗TMJDS的文献,运用统计软件对其中的数据进行分析,归纳出针灸治疗TMJDS的选穴规律。结果:共检索到相关文献231篇,最终纳入合格文献66篇,从中提取针灸处方84条,包含腧穴44个。针灸治疗TMJDS的常用腧穴包括下关、颊车、合谷等,足阳明胃经和手三阳经穴位使用频次最高,选穴部位多位于头面部和上肢部,交会穴、原穴和五输穴在特定穴的运用上占较大比例,高频穴位组合包括下关-颊车、下关-合谷、颊车-合谷等。结论:针灸治疗TMJDS符合循经取穴的基本原则,经脉选择主要集中于足阳明胃经和手三阳经,治疗重在通经活络,近端取穴与远端取穴相互配合。Objective:To explore the point selection regularity of acupuncture and moxibustion in treatment of temporomandibular joint disorder syndrome(TMJDS)through literature retrieval.Methods:The literature on treatment of TMJDS with acupuncture and moxibustion published from January 1979 to August 2018 were retrieved from Chinese National Knowledge Infrastructure(CNKI).Then the statistical software was used to analyze the data and summarize the point selection regularity of acupuncture and moxibustion in treatment of TMJDS.Results:Totally,231 relevant articles were retrieved,66 qualified articles were included,84 point prescriptions were extracted and 44 points were designed.The commonly used points in treatment of TMJDS with acupuncture and moxibustion included Xiaguan point(ST7),Jiache point(ST6),Hegu point(LI4),etc..The most frequently involved meridians were stomach meridian and three yang meridians.The points were mainly distributed in the head,face and upper limbs.In the application of special points,crossing points,yuan-primary points and five trans port points occupied a large proportion. The point combinations with high frequency included Xiaguan-Jiache,Xiaguan-Hegu,Jiache-Hegu,etc.. Conclusions:Treatments of TMJDS with acupuncture and moxibustion comply with the basic principle of selecting points along meridians. The most frequently involved meridians were stomach meridian and three yang meridians. With the emphasis on dredging meridians and activating collaterals,proximal and distal points were combined in the treatments.
