老年冠心病患者颈动脉造影内中膜厚度与胱抑素C、血流动力学变化的关系

摘　　要：目的:分析老年冠心病患者颈动脉造影内中膜厚度(IMT)与胱抑素C、血流动力学变化的关系。方法:选取郑州人民医院2016年2月至2019年2月收治的老年冠心病患者102例,使用美国GE公司生产的血管造影机进行检查,判断血管狭窄程度及内中膜的厚度。根据颈动脉造影IMT将102例患者分成内膜正常组30例、内膜增厚组62例、内膜斑块形成组10例。比较三组患者IMT、胱抑素C、最大血流速度、阻力指数,并对老年冠心病患者IMT影响因素进行多元逐步回归分析。结果:三组患者IMT厚度比较差异有统计学意义(P<0.05)。三组患者胱抑素C、最大血流速度及阻力指数比较差异有统计学意义(P<0.05)。胱抑素C、最大血流速度和阻力指数与IMT呈正相关性(P<0.05)。结论:老年冠心病患者颈动脉造影IMT指标与胱抑素C、血流动力学变化关系较密切,联合检查能指导临床判断老年冠心病病情,提升预后。Objective To investigate the relationship of intima-media thickness(IMT)measured by carotid angiography with cystatin C and hemodynamic changes in elderly patients with coronary heart disease.Methods A total of 102 patients with suspected coronary heart disease treated in the Zhengzhou People’s Hospital from February 2016 to February 2019 were enrolled.The degree of stenosis and the of all patients were determined by angiography machine made by American GE company.According to the Intima-media thickness measured by carotid angiography,102 patients were divided into normal intimal group(30 cases),intimal thickening group(62 cases)and intimal plaque formation group(10 cases).IMT,cystatin C,maximum blood flow velocity and resistance index were compared among the three groups,and the influencing factors of IMT in elderly patients with coronary heart disease were analyzed by multiple stepwise regression.Results There was a significant difference in IMT among the three groups(P<0.05).The levels of cystatin C,maximum blood flow velocity and resistance index were significantly different among the three groups(P<0.05).The levels of cystatin C,maximum blood flow velocity and resistance index were positively correlated with IMT(P<0.05).Conclusions Intima-media thickness measured by carotid angiography are closely related to cystatin C and hemodynamic changes in elderly patients with coronary heart disease,and the combined detection of those indexes can guide clinical judgment of elderly coronary heart disease and improve prognosis.
