解郁丸联合德谷胰岛素对糖尿病伴发抑郁症患者糖脂代谢及心理状态的影响

摘　　要：目的探讨解郁丸联合德谷胰岛素对糖尿病伴发抑郁症患者糖脂代谢及心理状态的影响。方法抽取濮阳市安阳地区医院2018年12月至2019年12月门诊和住院糖尿病伴发抑郁症患者80例,按照随机数字表法分为观察组与对照组,每组40例。两组患者均采取常规治疗,包括心理疏导、健康教育、控制饮食及适当运动等。对照组患者给予德谷胰岛素治疗,观察组在对照组的基础上结合解郁丸治疗。两组疗程均为8周。比较两组治疗疗效,治疗前后糖代谢、脂代谢、汉密尔顿抑郁量表(HAMD)-24项评分变化及不良反应。结果观察组总有效率(90.00%)高于对照组(67.50%,P<0.05)。两组治疗后空腹血糖(FPG)、糖化血红蛋白(HbA1c)和餐后血糖(2hPG)较治疗前降低(P<0.05);观察组治疗后FPG、HbA1c和2hPG低于对照组(P<0.05)。两组治疗后HDL-C较治疗前升高,而TC、LDL-C和TG较治疗前降低(P<0.05);观察组治疗后HDL-C高于对照组,而TC、LDL-C和TG低于对照组(P<0.05)。两组治疗后HAMD-24项评分较治疗前降低(P<0.05);观察组治疗后HAMD-24项评分低于对照组(P<0.05)。结论解郁丸联合德谷胰岛素治疗糖尿病伴发抑郁症患者疗效良好,可改善患者糖脂代谢,减轻抑郁症状。Objective To explore the effect of jieyu pill combined with insulin degludec on glucose and lipid metabolism and mental state in diabetic patients with depression.Methods The 80 diabetic patients with depression admitted to Anyang District Hospital of Puyang from December 2018 to December 2019 were divided into observation group and control group,with 40 cases in each group.The patients in the two groups received routine treatment,including psychological counseling,health education,diet control and proper exercise.The control group was treated with insulin degludec,the observation group was treated with jieyu pill on the basis of the control group.The course of treatment in both groups was 8 weeks.The therapeutic effect of the two groups was compared.The changes of glucose metabolism,lipid metabolism,Hamilton Depression Scale(HAMD)-24 scores and adverse reactions before and after treatment were compared.Results The total effective rate of the observation group(90.00%)was higher than that of the control group(67.50%,P<0.05).Fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c)and postprandial blood glucose(2hPG)in the two groups were lower than before treatment(P<0.05);FPG,HbA1c and 2hPG in the observation group were lower than those in the control group(P<0.05).after treatment,HDL-C increased,while TC,LDL-C and TG decreased(P<0.05);After treatment,HDL-C in the observation group was higher than that in the control group,while TC,LDL-C and TG were lower than those in the control group(P<0.05);after treatment,the scores of hamd-24 in the two groups were lower than those before treatment(P<0.05);after treatment,the scores of hamd-24 in the observation group were lower than those in the control group(P<0.05).Conclusions Jieyu pill combined with insulin degludec has a good therapeutic effect on diabetic patients with depression.It can improve the metabolism of glucose and lipid,and reduce the symptoms of depression.
