2017年广西柳州市常规免疫接种率监测结果评价分析

摘　　要：目的了解柳州市2017年常规免疫接种率监测工作情况,评价分析常规免疫接种率水平、接种率质量。方法选取柳州市2017年扩大免疫规划报告卡介苗(Bacille-Calmette-Guerin,BCG)、乙肝疫苗(hepatitis B vaccine,HepB)、脊髓灰质炎疫苗(poliomyelitis vaccine,PV,其中包括脊髓灰质炎灭活疫苗poliomyelitis inactivated vaccine,IPV、口服脊髓灰质炎二价减毒活疫苗bivalent live attenuated oral poliomyelitis vaccine,bOPV)、百白破疫苗(diphtheria,tetanus,pertussis vaccine,DTP)、麻风疫苗(measles,rubella vaccine,MR)和A群流脑疫苗(meningococcal polysaccharide vaccine-group A,MPV-A)常规免疫接种数据,采用估算接种率、D值法、R值法、脱漏率等方法对报告接种率的可靠性进行评价分析。结果柳州市2017年常规免疫接种率监测报告县、乡两级报告率均为100.00%;柳州市11县(区)BCG、HepB、PV、DTP和MPV-A疫苗报告接种率在98.62%~100.00%之间,估算接种率在63.06%~134.83%之间;D值评价,市级BCG为可疑,其余疫苗均可信;R值评价,市级3MR/HepB、3MR/PV、3MR/DPT均为可信;脱漏率评价,市级DTP、HepB和PV脱漏率均小于10%。结论柳州市常规免疫报告接种率保持在较高水平,但是接种率评价提示可能存在接种率虚高与接种不完整以及数据造假的问题。Objective To investigate the monitoring work of routine immunization rate in Liuzhou in 2017,and evaluate and analyze the level and quality of routine immunization coverage.Methods The routine immunization data of Bacille-Calmette-Guerin(BCG),hepatitis B vaccine(HepB),poliomyelitis(PV)[poliomyelitis inactivated vaccine(IPV)],[bivalent live attenuated oral poliomyelitis vaccine(bOPV)],Diphtheria,tetanus,pertussis vaccine(DTP),measles,rubella vaccine(MR)and meningitis vaccine-group A(MPV-A)were reported by EPI in Liuzhou City in 2017.The reliability of the reported coverage rate was evaluated and analyzed by the estimated coverage rates,D-value mathod,R-value method,leakage rate and so on.Results The rate of routine immunization in Liuzhou City in 2017 were 100.00%in both county and township,and the immunization rate of BCG,HepB,polio vaccine,DTP and MPV-A vaccine in 11 counties(districts)of Liuzhou City was between 98.62%and 100.00%,and the estimated coverage rate was between 63.06%and 134.83%.The D-value showed BCG was dubious,the rest of vaccines were credible.The R-value showed 3 BCG/HepB,3 BCG/polio vaccine and 3 BCG/DPT were all suspicious.The leakage rate of DTP,HepB and polio vaccine were lower than 10%.Conclusion The routine immunization coverage in Liuzhou was kept at a high level,but the vaccination evaluation rate indicated that there might be some problems,such as false high coverage rate,incomplete inoculation and data fraud.
