摘  要:目的评价激光照射对牙本质表面形态的改变,比较两种不同粘结剂和预备方法对牙本质粘结强度的影响。方法收集新鲜拔除的完整第三磨牙24颗,分为牙钻(A组)组和Er,Cr∶YSGG激光(B组)组两大组,两种预备方法暴露牙本质。再按照粘接剂不同随机分为Adper Easy One(A1、B1组)组;Single Bond Universal(A2、B2组)组,每组5颗牙齿;对照组(A3、B3组)组,每组2颗牙齿。其中,对照组在牙本质预备后不使用粘接剂处理。A3组和B3组使用扫描电子显微镜观察表面形态。其余各组使用树脂粘结后,制作哑铃型试件测试微拉伸粘结强度。采用两因素方差分析和组间比较进行统计学分析。结果扫描电镜检测结果显示牙钻组有明显玷污层;激光组表面不规则,干净,牙本质小管开放。牙钻组和激光组对牙本质微拉伸粘结强度的影响比较,差异无统计学意义(P>0.05);在牙钻组和激光组中,A2组对牙本质微拉伸粘结强度高于A1组;B2组对牙本质微拉伸粘结强度高于B1组,差异具有统计学意义(P<0.05)。结论Er,Cr∶YSGG激光预备牙本质可形成"酸蚀"样的表面形态,与牙钻预备相比对牙本质粘结强度无不利影响;预备后两种粘结剂均可与牙本质形成良好的粘结,其中Single Bond Universal的粘结效果更好。Objective To evaluate the morphology of laser-irradiated dentin and the effect of two different adhesives and preparation methods on dentin bond strength.Methods Twenty-four fresh extracted human third molars were used.They were divided into two groups which were prepared with Er,Cr:YSGG laser(Group A)or bur(Group B).The third molars of each group were further divided randomly into three groups by the adhesive used:Group A1/Group B1(Adper Easy One,n=5),Group A2/Group B2(Single Bond Universal,n=5)and Group A3/Group B3(no adhesive,n=2).The surface morphology of the third molars in Group A3 and B3 was observed with scanning electron microscope(SEM).The third molars in other groups were filled with resin and then cut into dumbbell-shaped specimens for microtensile bond strength test.The data collected were analyzed with two-way ANOVA and compared between groups.Results The SEM results showed that the third molars in the bur groups had obvious smear layer.On the other hand,as to the laser groups,the molar surface was irregular,no smear layer appeared and tubular opening was observed.There was no significant difference between the bur group and laser group in the impact on dentin microtensile bond strength(P>0.05).However,the differences between Group A2 and A1 and Group B1 and B2 in the impact were statistically significant(P<0.05),i.e.,Group A2>Group A1,Group B2>Group B1(P<0.05).Conclusions Er,Cr:YSGG laser will lead to the formation of"acid etch"on dentin surface.Unlike burs,Er,Cr:YSGG laser has no adverse effect on dentin Bond strength Both adhesives have good dentin bond performance,but Single Bond Universal is better.
