摘  要:目的分析肠道病毒71型(EV71)感染手足口病患儿Toll受体7(TLR7)、α干扰素(Interferon-α,IFN-α)、白细胞介素-6(Interleukin,IL-6)表达水平变化及TLR7基因多态性。方法选择2016年3月-2019年3月医院门诊和住院EV71感染手足口病患儿508例,其中无中枢神经系统并发症患儿327例作为轻症组,合并中枢神经系统并发症患儿181例作为重症组,选择同期在医院儿童保健科门诊体检儿童305名作为对照组。检测各组受试儿童TLR7、IFN-α以及IL-6 mRNA相对表达量,并分析TLR7 rs179019和rs3853839基因位点单核苷酸多态性。结果EV71病毒感染患儿TLR7 mRNA、IFN-αmRNA以及IL-6 mRNA表达均显著高于对照组(P<0.05),其中重症组患儿上述指标表达量均高于轻症组(P<0.05)。轻症组、重症组及对照组TLR7 rs179019基因位点基因型分布及C、A等位基因分布差异均无统计学意义,TLR7 rs3853839基因位点基因型及G、C等位基因分布比较差异具有统计学意义(P<0.05),其中重症组C等位基因分布率高于轻症组和对照组(P<0.05)。结论TLR7、IFN-α、IL-6参与EV71病毒感染过程中,可能是EV71感染并发展成为重症感染的关键因子,同时TLR7 rs3853839位点基因多态性与患儿EV71重症感染密切相关。OBJECTIVE To investigate the expression of Toll-like receptor 7(TLR7),interferon-α(IFN-α),interleukin-6(IL-6)and single nucleotide polymorphism of TLR7 gene in children with hand,foot and mouth disease(HFMD)infected by enterovirus 71.METHODS A total of 508 children with hand,foot and mouth disease(HFMD)infected by EV71 who were treated in outpatient department and were hospitalized from Mar 2016 to Mar 2019 were enrolled in the study,327 of whom did not have central nervous system complications and were assigned as the mild group,and 181 of who had central nervous system complications and were set as the severe group.Meanwhile,305 children who underwent physical examination in the outpatient department of pediatric health care in the hospital were chosen as the control group.The relative expression levels of TLR7,INF-alpha and IL-6 were detected,and the single nucleotide polymorphisms of TLR7 rs179019 and rs3853839 loci were analyzed.RESULTS The expression levels of TLR7 mRNA,IFN-αmRNA and IL-6 mRNA of the children with EV71 virus infection were significantly higher than those of the children in the control group(P<0.05),the expression levels of above indexes of the severe group were higher than those of the mild group(P<0.05).There were no significant differences in the distribution of genotypes of TLR7 rs179019 gene locus and the distribution of C/A allele among the mild group,the severe group and the control group;there were significant differences in the distribution of genotypes of TLR7 rs3853839 locus and the distribution of G/C allele(P<0.05);the distribution rate of C allele of the severe group was significantly higher than that of the mild group and the control group(P<0.05),and the distribution rate of C allele of the severe group was significantly higher than that of the mild group and the control group(P<0.05).CONCLUSION TLR7,IFN-alpha and IL-6 are involved in the process of EV71 infection,which may be the key factors for the progression of EV71 infection as severe infection,at the same time,the ge
