摘 要:目的了解湖北地区新生儿柯萨奇病毒(COX)、埃可病毒(ECHO)及EB病毒(EBV)感染情况。方法随机整群抽样法抽取2019年1月-2019年12月湖北省5所医院(武汉儿童医院、利川县人民医院、恩施民大医院、湖北省妇幼保健院、武大附属中南医院)住院疑似病毒感染新生儿400例,统计COX、ECHO及EBV病毒抗体阳性率,比较不同疾病、不同性别、不同季节新生儿抗体阳性率。结果400例新生儿血清样本共检出COX-IgM阳性35例(8.75%),ECHO-IgM阳性19例(4.75%),EBV-IgM阳性19例(4.75%);病毒性心肌炎和上呼吸道感染疾病COX-IgM抗体阳性率较高,上呼吸道感染疾病ECHO-IgM阳性率最高为8.96%,EBV-IgM抗体以自身免疫性疾病阳性率最高,其次为病毒性心肌炎;女性COX-IgM阳性率高于男性(18.37%vs 5.63%)(P<0.05);不同季节新生儿ECHO-IgM阳性率比较差异有统计学意义(P<0.05),ECHO-IgM阳性率秋季(11.43%)最高,其次为冬季(4.21%)。结论湖北地区新生儿COX-IgM阳性率最高,其次为EBV-IgM与ECHO-IgM,以上病毒感染与病毒性心肌炎、上呼吸道感染、哮喘、支气管肺炎及自身免疫性疾病发病密切相关,做好新生儿以上抗体筛查,可为感染相关疾病防控奠定基础。OBJECTIVE To understand the situation of Coxsackie virus(COX),Echo virus(ECHO)and EB virus(EBV)infections in newborns in Hubei Province.METHODS A total of 400 neonates with suspected viral infections who were hospitalized in 5 hospitals(Wuhan Children′s Hospital,Lichuan People′s Hospital,Hospital of Hubei Minzu University in Enshi,Hubei Maternal and Child Health Hospital,Central South Hospital Affiliated to Wuhan University)in Hubei Province from Jan 2019 to Dec 2019 were selected by random cluster sampling method.The positive rates of antibodies against COX,ECHO and EBV were statistically analyzed,and the positive rates of the antibodies were compared among the neonates with different diseases,the neonates of different genders and among the different seasons.RESULTS Among 400 serum samples collected from the neonates,there were 35(8.75%)tested positive for COX-IgM,19(4.75%)tested positive for ECHO-IgM and 19(4.75%)tested positive for EBV-IgM.The positive rate of COX-IgM antibody of the neonates with viral myocarditis and upper respiratory tract infectious diseases was relatively high,the positive rate of ECHO-IgM of the neonates with upper respiratory tract infectious disease was the highest(8.96%),and the positive rate of EBV-IgM of the neonates with autoimmune diseases was the highest,followed by the neonates with viral myocarditis.The positive rate of COX-IgM of the females was 18.37%,significantly higher than 5.63 of the males(P<0.05).There was significant difference in the positive rate of ECHO-IgM among the different seasons(P<0.05),the positive rate of ECHO-IgM was 11.43%in autumn(the highest),4.21%in winter.CONCLUSION The positive rate of COX-IgM of the neonates in Hubei province is the highest,followed by EBV-IgM and ECHO-IgM.COX-IgM,EBV-IgM and ECHO-IgM are closely associated with the viral myocarditis,upper respiratory tract infection,asthma,bronchopneumonia and autoimmune diseases.The screening of the above antibodies can lay a foundation for prevention and control of infection-related diseases.
