摘　　要：目的探讨B族链球菌感染所致脑膜炎患儿血清和脑脊液胰岛素样生长因子的表达水平变化。方法选取2014年1月1日-2019年2月28日江西省新余市妇幼保健院新生儿科44例B族链球菌感染的化脓性脑膜炎患儿为研究组,包括急性期和恢复期;正常新生儿23名为对照组,放射免疫分析法(Radioimmunoassay,RIA)检测入组研究对象血清和脑脊液中胰岛素样生长因子1(Insulin-like growth factor 1,IGF-1)和胰岛素样生长因子2(Insulin-like growth factor 2,IGF-2)的表达水平。结果研究组急性期血清IGF-1和IGF-2表达水平分别为(4.4±2.4)mg/L和(20.53±2.42)mg/L均低于正常对照组的(64.3±16.9)mg/L和(74.36±18.60)mg/L,两组比较差异有统计学意义(P<0.05),而脑脊液IGF-1和IGF-2表达水平(18.5±5.6)mg/L和(42.03±5.34)mg/L较正常对照组的(2.8±1.4)mg/L和(25.43±1.99)mg/L显著升高(P<0.05);与急性期相比,恢复期血清IGF-1和IGF-2表达水平显著升高(P<0.05),脑脊液IGF-1和IGF-2表达水平显著降低(P<0.05);恢复期血清和脑脊液IGF-1表达水平与正常对照组比较,差异无统计学意义。结论B族链球菌感染所致化脓性脑膜炎的新生儿的血清中胰岛素样生长因子的表达水平显著降低,脑脊液中胰岛素生长因子的表达水平显著升高。OBJECTIVE To investigate the expression of insulin-like growth factor(IGF)in serum and cerebrospinal fluid of neonates with meningitis caused by group B Streptococcus(GBS)infection.METHODS From Jan 1,2014 to Feb 28,2019,the neonates who were treated in the neonatal department of the Maternal and Child Health Hospital of Xinyu City were enrolled in the study,44 neonates with suppurative meningitis caused by GBS were assigned as the study group,including the acute phase and recovery phase;23 normal newborns were set as the control group.The levels of insulin-like growth factor 1(IGF-1)and insulin-like growth factor 2(IGF-2)in serum and cerebrospinal fluid were measured by radioimmunoassay(RIA).RESULTS The levels of serum IGF-1 and IGF-2 of the study group were respectively(4.4±2.4)mg/L and(20.53±2.42)mg/L during the acute phase,significantly lower than(64.3±16.9)mg/L and(74.36±18.60)mg/L of the control group(P<0.05);the levels of cerebrospinal fluid IGF-1 and IGF-2 of the study group were respectively(18.5±5.6)mg/L and(42.03±5.34)mg/L,significantly higher than(2.8±1.4)mg/L and(25.43±1.99)mg/L of the control group(P<0.05).The levels of serum IGF-1 and IGF-2 were significantly higher during the recovery phase than during the acute phase(P<0.05);the levels of cerebrospinal fluid IGF-1 and IGF-2 were significantly lower during the recovery phase than during the acute phase(P<0.05).There were no significant differences in the levels of serum and cerebrospinal fluid IGF-1 between the study group and the control group during the recovery phase.CONCLUSION The level of serum IGF of the neonates with suppurative meningitis caused by GBS is significantly reduced,and the levels of cerebrospinal fluid IGF is significantly elevated.
