摘　　要：目的探讨近端胃切除加幽门成形术对近端胃癌患者术后恢复的影响。方法选择我院因近端胃癌拟行近端胃切除的72例患者,将其随机分为对照组(36例,近端胃切除术)和观察组(36例,近端胃切除+幽门成形术)。比较两组患者术后3个月的食管反流情况、胃镜检查结果及营养指标。结果观察组的食管反流严重程度与胃镜下反流性食管炎分级均低于对照组(P<0.05)。观察组的血红蛋白、血浆白蛋白水平及体质量均高于对照组(P<0.05)。结论近端胃切除加幽门成形术能够降低患者术后反流性食管炎的发生率,并改善其营养状况。Objective To investigate the effect of proximal gastrectomy and pyloroplasty on postoperative recovery of patients with proximal gastric cancer.Methods A total of 72 patients with proximal gastric cancer who were scheduled to undergo proximal gastrectomy in our hospital were selected,and the patients were randomly divided into control group(36 cases,proximal gastrectomy)and observation group(36 cases,proximal gastrectomy+pyloroplasty).The esophageal reflux,gastroscopy result and nutritional indexes at 3 months after operation were compared between the two groups.Results The order of severity degree of esophageal reflux and grading of reflux esophagitis under gastroscope in the observation group were lower than those in the control group(P<0.05).The levels of hemoglobin,plasma albumin and body mass in the observation group were higher than those in the control group(P<0.05).Conclusion Proximal gastrectomy and pyloroplasty can reduce the incidence of reflux esophagitis and improve the nutritional status of patients after surgery.
