早期主动活动方案在胸腔镜肺叶切除术后机械通气患者中的应用

摘　　要：目的评价胸腔镜肺叶切除术后机械通气患者早期主动活动的效果。方法便利抽样选取2018年1一12月入住某院重症监护病房(intensive care unit,TCU)行机械通气的胸腔镜肺叶切除术后患者,按时间先后分为两组。对照组予术后常规护理,观察组在此基础上予以早期主动活动方案。比较两组患者肌力、下肢静脉血栓发生率、不良事件发生率、机械通气时间、吸痰次数、ICU住院天数及术后住院天数等情况。结果观察组肌力提高程度高于对照组,下肢深静脉血栓发生率,机械通气时间、吸痰次数、ICU住院天数和术后住院天数均低于对照组,两组比较差异均有统计学意义(均P<0.05)。结论行机械通气的胸腔镜肺叶切除术后患者进行早期主动活动安全可行,有利于提高患者肌力,缩短机械通气时间。Objective To evaluate the effect of early ambulation in patients with mechanical ventilation af-ter thoracoscopic lobectomy.Methods By the convenience sampling method,the patients who underwentmechanical ventilation after thoracoscopic lobectomy from January to December 2018 were selected andrandomly divided into two groups.The control group was given routine nursing care after the operation,andthe observation group received early ambulation on this basis.Muscle strength,incidence of lower extremityvenous thrombosis,incidence of adverse events,duration of mechanical ventilation,times of sputum aspira-tion,Intensive Care Unit(ICU)stay and postoperative hospital stay were compared between the twogroups.Results The improvemnent of muscle strength in the observation group was higher than that in thecontrol group,while the incidence of deep venous thrombosis,mechanical ventilation time,sputum suctiontimes,ICU hospitalization days and postoperative hospitalization days in the observation group were lowerthan those in the control group(all P<0.05).Conclusions Early ambulation after thoracic surgery wassafe and feasible,which can improve the muscle strength of patients and shorten the time of mechanicalventilation.
