基于吞咽喉镜的吞咽摄食训练在下咽癌术后患者中的应用

摘　　要：目的探讨以吞咽喉镜为指导的吞咽摄食训练对改善下咽癌术后患者吞咽功能的效果。方法便利抽样选取某院20例下咽癌术后患者,运用吞咽喉镜对其进行客观吞咽功能评估,直接观察患者咽部痰液聚积,误吸、食物残留情况,并根据结果进行针对性的吞咽摄食训练。训练后3个月再次行吞咽喉镜检查,比较训练前后吞咽功能的变化。结果吞咽喉镜检查显示,20例患者训练前痰液聚积18例、误吸16例.食物残留18例,训练后术后3个月复查分别降低为4例、6例和3例,较训练前明显好转(均P<0.05)。结论吞咽喉镜可以作为下咽癌患者评估吞咽功能的有效方法,并能够提供精准的吞咽摄食指导,帮助改善患者吞咽功能。Objective To explore the effect of swallowing-feeding training guided by fiberoptic endoscopicevaluation of swallowing(FEES)in postsurgical patients with hypopharyngeal carcinoma(HPC).MethodsObjective evaluation of the swallowing function was carried out in 20 patients with HPC after operation.Sputum accumulation,aspiration and food residue were observed directly;swallowing and feeding trainingwas conducted according to the results.Three months after operation,the patients were examined again bylaryngoscope,and the swallowing function was compared before and after training.Results FEES resultsshowed that sputum accumulation in 18 cases,aspiration in 16 cases and food residue in 18 cases beforetraining were reduced to 4 cases,6 cases and 3 cases respectively after 3 months of training,which were sig-nificantly improved compared with those before training(all P<0.05).Conclusions.FEES can be used asan effective tool for evaluating swallowing function in HPC patients and contribute to making individualizedguidance for swallowing-feeding training to improve the swallowing function of HPC postsurgical patients.
