摘 要:目的探讨中晚孕应用超声评估胎儿生长受限及预测其出生结局的价值。方法选取2015年4月至2018年3月锦州市妇婴医院收治的86例胎儿生长受限孕妇,根据新生儿出生结局进行分组,将出生健康者视为对照组(40例),将出生异常者视为研究组(46例)。两组均使用超声评估,对比大脑中动脉、脐动脉的血流参数变化,并进行统计学分析。结果两组脐动脉阻力指数、大脑中动脉阻力指数及二者比值比较,差异有统计学意义(P<0.05)。对照组未发生脐动脉舒张末期的血流缺失或者反向(AREDF),大脑中动脉阻力指数/脐动脉阻力指数>1,分娩孕周平均为(36.54±0.98)周。研究组5例发生AREDF,41例大脑中动脉阻力指数/脐动脉阻力指数<1,分娩孕周平均为(31.54±1.54)周。脐动脉阻力指数的ROC曲线下面积为0.927,大脑中动脉阻力指数/脐动脉阻力指数的ROC曲线面积为0.984。结论大脑中动脉阻力指数、大脑中动脉阻力指数/脐动脉阻力指数、脐动脉阻力指数均可用于胎儿生长受限的评估与出生结局预测,其中大脑中动脉阻力指数/脐动脉阻力指数的应用价值最高。Objective To investigate the value of ultrasonography in evaluating fetal growth limitation and predicting birth outcomes in late pregnancy.Methods The study involved 86 pregnant women with limited fetal growth.The study time was from April 2015 to March 2018.According to the birth outcome of the newborn,the birth healthy people were regarded as reference groups.A total of 40 cases were treated as research groups,and 46 cases were treated as abnormal births.All use ultrasound evaluation.The changes of blood flow parameters in middle cerebral artery and artery were analyzed statistically.Results There was significant difference between the two groups(P<0.05).In the control group,there was no end diastolic blood flow loss or reversal(AREDF),middle cerebral artery resistance index/umbilical artery resistance index>1,and the average gestational week of delivery was(36.54±0.98)weeks.In the study group,5 cases had AREDF,41 cases had middle cerebral artery resistance index/umbilical artery resistance index<1,and the average gestational week of delivery was(31.54±1.54)weeks.The area under the ROC curve of the umbilical artery resistance index was 0.927,and the ROC curve area of the middle cerebral artery resistance index/umbilical artery resistance index was 0.984.Conclusion The arterial resistance index in brain,the artery resistance index in brain,the umbilical artery resistance index and the umbilical artery resistance index can be used to evaluate fetal growth restriction and predict the outcome of birth.The application value of the arterial resistance index in brain and umbilical artery resistance index is the highest.
