老年亚临床甲状腺功能减退症患者左旋甲状腺素治疗前后甲状腺功能、血脂水平改善情况

摘　　要：目的探讨老年亚临床甲状腺功能减退症患者左旋甲状腺素治疗前后甲状腺功能、血脂水平改善效果。方法选取2017年2月至2018年11月纳入我院治疗的80例老年亚临床甲状腺功能减退症患者,采用随机数字表进行分组。常规药物治疗组采取常规方法治疗,左旋甲状腺素辅助治疗组则采取常规方法加上左旋甲状腺素治疗,左旋甲状腺素(优甲乐),由德国默克公司生产。比较常规药物治疗组、左旋甲状腺素辅助治疗组疗效,血脂指标(LDL-C、TC和TG),治疗前后患者甲状腺生化指标(血清游离三碘甲腺原氨酸、血清游离四碘甲腺原氨酸以及促甲状腺激素),以及不良反应。结果左旋甲状腺素辅助治疗组相比较常规药物治疗组的疗效较高,P<0.05。治疗前常规药物治疗组、左旋甲状腺素辅助治疗组患者的血清游离三碘甲腺原氨酸、血清游离四碘甲腺原氨酸以及促甲状腺激素相近,P>0.05;治疗后左旋甲状腺素辅助治疗组患者的血清游离三碘甲腺原氨酸、血清游离四碘甲腺原氨酸以及促甲状腺激素变化幅度更大,P<0.05。结论常规方法加上左旋甲状腺素治疗老年亚临床甲状腺功能减退症的效果理想,可有效改善患者的甲状腺功能、血脂水平,且无明显不良反应,安全性高。Objective To explore the effect of improving thyroid function and blood lipid levels in elderly patients with subclinical hypothyroidism before and after levothyroxine treatment.Methods A random number table was used to group 80 elderly patients with subclinical hypothyroidism who were admitted to our hospital from February 2017 to November 2018.The conventional drug treatment group was treated with conventional methods,and the levothyroxine adjuvant treatment group was treated with conventional methods plus levothyroxine treatment.Levothyroxine(U-Gal),produced by Merck,Germany.Compare the curative effect of conventional drug treatment group and levothyroxine adjuvant treatment group,blood lipid indexes(LDL-C,TC and TG),thyroid biochemical indexes of patients before and after treatment(serum free triiodothyronine,serum free tetraiodothyronine and thyroid stimulating hormone),and adverse reactions.Results The levothyroxine adjuvant therapy group had higher efficacy than the conventional drug therapy group,P<0.05.Before treatment,the thyroid biochemical indexes of patients in the conventional drug treatment group and the levothyroxine adjuvant treatment group were similar in serum free triiodothyronine,serum free tetraiodothyronine and thyroid stimulating hormone,P>0.05;after treatment,L-thyronine the thyroid biochemical indexes of patients in the adjuvant thyroxine treatment group had greater changes in serum free triiodothyronine,serum free tetraiodothyronine and thyroid-stimulating hormone,P<0.05.Conclusion Conventional methods and levothyroxine are effective in the treatment of subclinical hypothyroidism in the elderly.They can effectively improve the patients'thyroid function and blood lipid levels,without obvious adverse reactions,and high safety.
