ICU护士对慢性阻塞性肺疾病机械通气患者早期活动护理现况及其影响因素分析

摘　　要：目的调查昆山市二级以上医院ICU护士对慢性阻塞性肺疾病(COPD)机械通气患者早期活动的护理现状,并分析其影响因素,为COPD机械通气患者开展早期活动提供参考依据。方法采用自行设计的《ICU护士对COPD机械通气患者实施早期活动护理知识、态度、行为调查表》对昆山市二级以上医院重症医学科123名护士进行调查,收集一般资料,采用单因素方差分析,比较不同特征ICU护士对COPD机械通气患者早期活动的知识、信念和行为得分差异,并采用逐步多元线性回归分析其影响因素。结果ICU护士对COPD机械通气患者早期活动实践行为均较差,总分为(40.22±7.12)分。单因素方差分析显示,职称、学历、是否为专科护士、年资及能级是ICU护士对COPD机械通气患者早期活动知识、态度、行为的影响因素(P<0.05);多元线性回归分析显示,能级及年资是ICU护士对COPD机械通气患者早期活动知识、态度及行为的独立预测因素(P<0.05)。结论ICU护士对COPD机械通气患者早期活动实践行为处于较低水平,有待于进一步提高。对于能级及年资低的护士应加强COPD机械通气患者早期活动知识、态度及行为专业临床培训和教育,形成长效机制,促进早期活动在临床上顺利开展,促进患者早日康复,改善预后。Objective To investigate the current status of nursing care of patients with COPD mechanical ventilation in ICU nurses in the second-level hospitals of Kunshan,and analyze the influencing factors to provide reference for early activities of COPD mechanical ventilation patients.Methods A selfdesigned(Questionnaire on Knowledge,Attitude and Behavior of ICU Nurses in Implementing Early Activity Nursing for Patients with COPD Mechanical Ventilation)was used to investigate 123 nurses in the critical care department of kunshan second-level and above hospitals.General data were collected and univariate analysis of variance was used to compare the differences in knowledge,beliefs and behavior scores of ICU nurses with different characteristics in patients with early COPD mechanical ventilation,and stepwise multivariate linear regression was used to analyze the influencing factors.Results ICU nurses had a poor performance in the early activities of patients with COPD mechanical ventilation,with a total score of(40.22±7.12).One-way analysis of variance showed that the professional title,education,whether it was a specialist nurse,seniority and energy level were the influencing factors of ICU nurses'knowledge,attitude and behavior in early activities of COPD mechanical ventilation patients(P<0.05);multiple linear regression analysis showed that energy level and seniority were independent predictors of ICU nurses'knowledge,attitudes,and behaviors in early COPD patients(P<0.05).Conclusion ICU nurses'early activity and practice behaviors of COPD patients with mechanical ventilation are at a low level,which needs to be further improved.Nurses with low energy levels and years of experience should strengthen professional clinical training and education on knowledge,attitude and behavior of early COPD patients with mechanical ventilation,so as to form a long-term mechanism,promote the smooth development of early activities in clinical practice,promote the early recovery of patients,and improve the prognosis.
