纤维乳管镜在乳腺导管内囊性增生病变诊断及治疗中的应用

摘　　要：目的探讨纤维乳管镜在乳腺导管内囊性增生病变诊断及治疗中的应用价值.方法选取临床初步诊断为乳腺导管内囊性增生病变患者560例,随机分为实验组与对照组,每组280例.实验组应用乳管镜进行检查和治疗,对发现乳管内隆起性病变患者予以乳管镜联合定位针标记,实施手术治疗;对乳管镜下明确为乳腺导管内囊性增生病变实施乳管镜灌注冲洗治疗.对照组单纯实施常规药物治疗,观察疗效.结果实施乳管镜检查的280例患者中有27例乳管内隆起性病变,联合定位针实施手术治疗,病理诊断为乳腺癌患者6例,乳腺肿物和乳管内癌检出率均高于对照组,差异具有统计学意义(P<0.05);其余253例乳腺导管内囊性增生病变患者均实施乳管灌注冲洗治疗,临床治愈率为64.82%,总有效率为89.72%,二者均高于对照组,差异具有统计学意义(P<0.01).结论纤维乳管镜在乳腺导管内囊性增生病变诊断及治疗方面均具有较高的临床应用价值,可早期发现乳腺癌.Objective To evaluate the value of fiberoptic ductoscopy in the diagnosis and treatment of intraductal cystic hyperplasia of breast.Method A total of 560 patients with intramedullary cystic hyperplasia of breast were selected and randomly divided into experimental group and control group,280 cases each group.The patients in the experimental group were examined and treated with lactofiberoptic ductoscopy.The patients combined lactofiberoptic ductoscopy with localization needle labeling were treated with surgery;The breast intraductal cystic hyperplasia was treated with endoscopic perfusion and irrigation.The control group was treated with conventional drugs and the curative effect was observed.Results A total of 280 patients were examined by dendoscopy,27 patients with indotal hyperplasia of the breast were found,and 6 patients were pathologically diagnosed as breast cancer.The detection rate of breast mass and intraluctal carcinoma were higher than those of the control group,and the difference was statistically significant(P<0.05);The remaining 253 patients with inductal cystic hyperplasia of the breast were treated with lactoductal irrigation.The clinical cure rate was 64.82%and the total effective rate was 89.72%,both of which were higher than the control group,and the difference was statistically significant(P<0.01).Conclusion Fiberoptic ductoscopy has high clinical appli-cation value in diagnosis and treatment of intraductal cystic hyperplasia of breast,and can detect breast cancer early.
