气道管理方案在肺结核并发呼吸衰竭患者中的实施效果评价

摘　　要：目的回顾性分析2017年1月至2018年6月在首都医科大学附属北京胸科医院重症医学科重症加强护理病房(intensivecare unit,ICU)接受机械通气治疗的56例肺结核并发呼吸衰竭患者的呼吸机相关性肺炎发生情况、护理不良事件发生情况及治疗转归情况。在56例患者中,7.1%(4/56)发生呼吸机相关性肺炎;12.5%(7/56)发生与气管插管相关的不良事件,均得到及时妥善处理;未出现呼吸道黏膜千燥和气道堵塞的患者。32例患者好转后转出ICU,抢救成功率为57.1%;死亡24例,死亡率为42.9%。研究认为,对肺结核并发呼吸衰竭患者采取气道管理方案,患者并发症发生率较低,对于改善患者预后有一定促进作用。Fifty-six patients with pulmonary tuberculosis complicated with respiratory failure who were treated in the Intensive Care Unit of Bejjing Chest Hospital,Capital Medical University from January 2017 to June 2018 were retrospectively analyzed,and to carry out airway management program and analyze the incidence of complications rclated to mechanical ventilation,nursing adverse events and treatment outcome.Among the 56 patients,7.1%(4/56)were complicated with ventilator-associated pneumonia;the rate of adverse events related to endotracheal intubation was 12.5%(7/56),all responded promptly and properly;no case occurred respiratory tract mucosa dryness and airway blockage.Thirty-two patients transferred out of ICU after improvement,the rescue success rate was 57.1%;twenty-four cases died,the mortality rate was 42.9%.Our study show that airway management program for pulmonary tuberculosis patients complicated with respiratory failure has lower incidence of complications,and which has a.certain promoting effect on improving prognosis.
