改良手指引导穿刺法建立腹膜前间隙在腹腔镜全腹膜外疝修补术中的应用

摘　　要：目的:探究在腹腔镜全腹膜外疝修补术(TEP)中采用改良手指引导穿刺法建立腹膜前间隙的体会。方法:2018年1月-2019年6月腹股沟疝患者120例,所有患者均行TEP术,术中均成功采用改良手指引导穿刺法建立腹膜前间隙。结果:建立腹膜前间隙平均时间(9.80±2.30)min,未出现血管损伤,2例在穿刺过程中出现腹膜破损,但仍顺利完成手术,118例完成腹膜前间隙的建立。术中出血量(3.01±0.62)ml,术后发生局部血肿8例,术后住院时间(3.80±0.53)d,均痊愈出院。随访6个月以上,无复发病例。结论:TEP中采用改良手指引导穿刺法建立腹膜前间隙应用常规器械,操作迅速,建立空间安全、快捷、有效。Objective:To explore the experience of using inproved finger guided puncture to establish preperitoneal space in laparoscopic total extraperitoneal herin repair.Method:From January 2018 to June 2019,120 cases of inguinal hernia patients were completed,all underwent TEP surgery,during which improved finger guided puncture was successfully used to establish preperitoneal space.Result:The mean time to establish the preperitoneal space was (9.80±2.30) min,and no vascular damage occurred.Two cases had peritoneal damage during the puncture,but the operation was still completed successfully,and 118 cases had completed the establishment of the preperitoneal space.Intraperative blood loss (3.01±0.62) ml,local hematoma occurred in 8 cases,postoperative hospital stay (3.80±0.53) days,all recoved and discharged follow-up for more than 6 months showed no recurrence.Conclusion:During TEP,improved finger guided puncture is used to establish preperitoneal space,which is operated quickly with conventional instruments,and the space is safe,fast and effective.
