血清铁蛋白、T淋巴细胞亚群水平变化与小儿急性白血病发病的相关性

摘　　要：目的探讨急性白血病患儿血清铁蛋白、T淋巴细胞亚群水平变化,并分析其临床意义。方法选取急性白血病患儿为研究对象,根据其病情分为初发组、复发组及缓解组,选取健康儿童作为对照。观察4组血清铁蛋白、T淋巴细胞亚群、C反应蛋白、降钙素原水平的差异,分析急性白血病患儿血清铁蛋白与T淋巴细胞亚群、C反应蛋白、降钙素原水平的相关性。结果4组血清SF、CRP和PCT水平由高到低分别为复发组、初发组、缓解组和对照组;4组IL-4、IL-6和TNF-α水平由高到低分别为复发组、初发组、缓解组和对照组。急性白血病组患儿的CD3^+、CD4^+、CD4^+/CD8^+低于对照组,CD8^+水平高于对照组。急性白血病患儿的SF水平与IL-4、IL-6、TNF-α、CRP、PCT和CD8^+水平正相关,与CD3^+、CD4^+、CD4^+/CD8^+水平负相关。结论急性白血病患儿的血清铁蛋白较高,T淋巴细胞亚群整体水平较低,且两者间密切相关。Objective To observe the changes of serum ferritin and T lymphocyte subsets in children with acute leukemia,and to analyze its clinical significance.Methods The children with acute leukemia were selected as the research objects.According to their condition,they were divided into primary group,recurrent group and refractory group.Healthy children were selected as the control group.The differences of serum ferritin,T lymphocyte subsets,C reactive protein and procalcitonin levels of the 4 groups were observed,and the correlation between serum ferritin and T lymphocyte subsets,C reactive protein and procalcitonin levels in children with acute leukemia was analyzed.Results The levels of serum SF,CRP and PCT in the four groups were higher than those in the recurrence group,the primary group,the remission group and the control group.The levels of IL-4,IL-6 and TNF-alpha in the 4 groups were higher than those in the recurrence group,the initial group,the remission group and the control group;The levels of CD3^+,CD4^+and CD4^+/CD8^+in the children with acute leukemia were lower than those of the control group,and the level of CD8^+was higher than that of the control group;the levels of SF in children with acute leukemia were positively correlated with the levels of IL-4,IL-6,TNF-,CRP,PCT and CD8^+,and negatively correlated with the levels of CD3^+,CD4^+and CD4^+/CD8^+.Conclusion The serum ferritin level is high in children with acute leukemia,and the overall level of T lymphocyte subsets is low,and the 2 are closely related.
