摘　　要：目的:探讨不同固定方式对桡骨远端粉碎骨折患者影像学结果及腕关节功能的影响。方法:选取2018年1月-2019年12月就诊于笔者所在医院的86例桡骨远端粉碎骨折患者,按随机数字表法分为试验组(43例)与对照组(43例)。对照组行外固定架结合克氏针有限固定,试验组行掌侧锁定板内固定。对比两组影像学结果、腕关节活动度、腕关节功能及术后并发症发生情况。结果:术后3个月,试验组尺偏角、掌倾角,腕关节背伸、尺偏、桡偏、旋前、旋后、掌屈角度大于对照组,桡骨高度及腕关节优良率高于对照组,差异均有统计学意义(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:掌侧锁定板内固定治疗桡骨远端粉碎骨折与外固定架结合克氏针有限固定相比安全性相当,但前者可获得更好的影像学结果,提高腕关节活动度,改善腕关节功能。Objective:To investigate the effect of different fixation methods on imaging results and wrist joint function in patients with comminuted fracture of distal radius.Method:A total of 86 patients with comminuted distal radius fractures who were treated in our hospital from January 2018 to December 2019 were selected and divided into the test group (43 cases) and the control group (43 cases) according to the random number table method.The control group was fixed by external fixation combined with Kirschner wire,and the test group was fixed by palmar locking plate.The imaging results,wrist joint mobility,wrist joint function and postoperative complications were compared between the two groups.Result:At 3 months after operation,the ulnar deviation,palmar inclination,wrist extension,ulnar deviation,radial deviation,pronation,supination,palmar flexion angle in the test group were bigger than those in the control group,and the radial height and wrist excellent rate were higher in the test group than those in the control group,the differences were statistically significant (P<0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups (P>0.05).Conclusion:The internal fixation of the volar locking plate for the treatment of comminuted fractures of the distal radius is comparable to external fixation combined with limited fixation with Kirschner wire,but the former can obtain better imaging results,improve wrist mobility,and improve wrist function.
