摘 要:目的:分析突发性聋患者血清超敏C反应蛋白、中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值的变化,探讨其与突发性聋发病的关系。方法:回顾性分析笔者所在医院2016年1月-2019年12月收治的资料齐全的突发性聋患者88例为突聋组,收集同期住院的声带息肉、瘢痕疙瘩及鼻中隔偏曲患者88例作为对照组,记录所有患者的血清超敏C反应蛋白(hs-CRP)、中性粒细胞绝对值、淋巴细胞绝对值、血小板计数、血小板比容。通过计算得到中性粒细胞与淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)及血小板与淋巴细胞比值(platelet-to-lymphocyte ratio,PLR)。采用二项Logistic回归分析突发性耳聋的影响因素。结果:突聋组的中性粒细胞绝对值、hs-CRP、NLR、PLR高于对照组,淋巴细胞绝对值低于对照组,差异均有统计学意义(P<0.05),二项Logistic回归分析结果显示淋巴细胞绝对值升高是突发性聋发病的保护因素,hs-CRP、NLR升高是突发性聋发病的危险因素(P<0.05)。结论:炎症反应参与突发性聋的发病过程,hs-CRP及NLR升高是突发性聋发病的危险因素。Objective:To analyze the difference of serum high sensitivity C-reactive protein,neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio in patients with sudden deafness and explore their relationship with the occurence of sudden deafness.Method:From January 2016 to December 2019,88 patients with sudden deafness admitted to our hospital were retrospectively analyzed as the sudden deafness group,88 patients with poly of vocal cord,keloids,or nasal septum deviation during the same period were recorded as the control group.The patients serum high-sensitivity C-reactive protein (hs-CRP),absolute neutrophil count,absolute value of lymphocyte,platelet count and platelet volume in blood routine were recorded.The neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) were obtained by calculation.Compared the data of two groups and binary Logistic regression analyses were performed to observe the influence factor.Result:The absolute values of neutrophils,hs-CRP,NLR,and PLR in the sudden deafness group were higher than those in the control group,and the absolute values of lymphocytes were lower than those in the control group,the difference was statistically significant (P<0.05).The results of binary Logistic regression analysis showed increased absolute value of lymphocytes was a protective factor for sudden deafness,and elevated hs-CRP and NLR were risk factors for sudden deafness (P<0.05).Conclusion:Inflammatory response is involved in the pathogenesis of sudden deafness.Increased hs-CRP and NLR are risk factors for sudden deafness.
