靳三针联合重复经颅磁刺激对精神发育迟缓患者认知发育及脑电活动的影响

摘　　要：目的观察靳三针头穴留针联合重复经颅磁刺激(rTMS)对精神发育迟缓(MR)儿童认知发育的影响,为MR患者的治疗寻求一种更为有效的治疗方法。方法将75例精神发育迟缓患者按照1:1:1的比例随机分为对照A组(25例)、对照B组(25例)和观察组(25例),3组在接受常规康复训练基础上分别接受靳三针头穴留针治疗、重复经颅磁刺激、靳三针头穴留针联合重复经颅磁刺激治疗。所有对象于治疗前及治疗3个月后,进行韦氏幼儿智力量表(WISC)、事件相关电位听觉P300、脑电图检测,以观察患者治疗前后智力及脑电活动改善情况。结果治疗后3组患者WISC评分、P300的潜伏期均提高(P<0.05);P300的波幅均降低(P<0.05);对照A组及观察组双侧额区、双侧颞区脑电活动明显改善(P<0.05),对照B组双侧额区、左侧颞区脑电活动明显改善(P<0.05),其中以观察组效果最佳,优于其他两组(P<0.05)。结论在常规康复训练基础上,靳三针头穴留针联合rTMS可以有效提高MR患者智能发育水平及改善脑电活动。Objective To observe the effect of Jin’s three needle therapy with needles retained at the scalp acupoints plus repetitive transcranial magnetic stimulation(rTMS)on cognitive development in children with mental retardation(MR),for seeking a more effective treatment method for kids with MR.Method Seventy-five MR patients were randomized into control group A(25 cases),control group B(25 cases)and an observation group(25 cases)at 1:1:1.In addition to the routine rehabilitation training,the three groups respectively received Jin’s three needle therapy with needles retained at the scalp acupoints,rTMS and Jin’s three needle therapy with needles retained at the scalp acupoints plus rTMS.All the subjects were evaluated using the Wechslar Intelligence Scale for Children(WISC),auditory event-related potential P300 and electroencephalogram(EEG)before treatment and after 3-month treatment,to observe the improvements in the intelligence and brain electrical activity in the patients after intervention.Result The WISC score and P300 latency increased after treatment in the three groups(P<0.05);the P300 amplitude decreased in the three groups(P<0.05);the brain electrical activities in bilateral frontal and temporal zones showed significant improvements in control group A and the observation group(P<0.05),and significant improvements were found in bilateral frontal and left temporal zones in control group B(P<0.05),while the observation group showed the most significant improvement,superior to the other two groups(P<0.05).Conclusion In addition to the routine rehabilitation training, Jin’s three needle therapy with needles retained at the scalp acupoints plus rTMS can effectivelyenhance the intelligence development level and improve the brain electrical activity in MR patients.
