杵针治疗腰椎术后慢性腰痛的疗效观察

摘　　要：目的观察杵针治疗腰椎术后慢性腰痛的临床疗效及腰部红外热像变化。方法将200例腰椎术后慢性腰痛患者随机分为治疗组和对照组,每组100例。治疗组采用金刚杵寻按和点叩腰阳关八阵穴治疗,对照组采用常规电针治疗。观察两组治疗前后视觉模拟量表(VAS)评分、疼痛分级指数(PRI)评分、现有疼痛强度(PPI)评分及腰部红外热像温度的变化情况,并比较两组临床疗效。结果两组治疗后各项评分(VAS评分、PRI评分、PPI评分)及腰部红外热像温度与同组治疗前比较差异均具有统计学意义(P<0.05)。治疗组治疗后各项评分及腰部红外热像温度与对照组比较差异均具有统计学意义(P<0.05)。治疗组总有效率和愈显率分别为90.0%和58.0%,对照组分别为82.0%和37.0%。两组愈显率比较差异具有统计学意义(P<0.05)。结论杵针治疗可有效缓解腰椎术后慢性腰痛,提升腰部肌肉温度。Objective To observe the clinical efficacy of pestle acupuncture in treating chronic low back pain after lumbar spinal surgery and the change in infrared thermal image of the low back.Method A total of 200 patients with chronic low back pain after lumbar spinal surgery were randomized into a treatment group and a control group,with 100 cases in each group.The treatment group was intervened by Jin Gang Chu(a kind of pestle needle shaped like the weapon symbolizing the power of wisdom)to press and tap the eight acupoints taking Yaoyangguan(GV3)as the center,while the control group was treated with ordinary electroacupuncture.The scores of Visual Analogue Scale(VAS),Pain Rating Index(PRI)and Present Pain Intensity(PPI)and the infrared thermal image temperature of the low back were observed before and after treatment in the two groups.The clinical efficacy was also compared between the two groups.Result The score of each item(VAS,PRI and PPI scores)and the infrared thermal image of the low back changed significantly after treatment in both groups(P<0.05).After intervention,the score of each item and the infrared thermal image temperature in the treatment group were significantly different from those in the control group(P<0.05).The total effective rate was 90.0%and the markedly effective rate was 58.0%in the treatment group,versus 82.0%and 37.0%in the control group.There was a significant difference in the markedly effective rate between the two groups(P<0.05).Conclusion Pestle acupuncture can effectively mitigate the chronic low back pain after lumbar spinal surgery;it can increase the temperature of the muscles in the low back.
