摘 要:目的观察不同频率电针对膜迷路积水豚鼠耳蜗形态及血浆血管加压素(AVP)浓度变化的影响,优选出电针治疗膜迷路积水适宜参数,初步探讨电针治疗梅尼埃病可能机制。方法健康雄性豚鼠40只随机分为空白组、模型组、电针组(据不同频率分为3个亚组),每组8只。除空白组,其余各组均通过腹腔注射醋酸去氨加压素建立膜迷路积水模型。空白组不予干预;模型组造模后采取与电针组相同固定方式;电针组各个亚组取百会和听宫穴,分别采用2 Hz、15 Hz、2/15 Hz3种不同频率进行治疗。干预结束后检测耳蜗积水程度,并计算R值,检测血浆AVP浓度。结果与空白组比较,模型组豚鼠耳蜗R值增加(P<0.01);与模型组比较,各电针组豚鼠耳蜗R值均降低(P<0.05);各电针组比较,2/15 Hz电针组较2 Hz电针组、15 Hz电针组降低,差异有统计学意义(P<0.05);2 Hz电针组、15 Hz电针组比较差异无统计学意义(P>0.05)。与空白组比较,模型组豚鼠血浆AVP浓度增加(P<0.01);与模型组比较,各电针组豚鼠血浆AVP浓度均降低(P<0.05);各电针组比较,2/15 Hz电针组较2 Hz电针组、15 Hz电针组降低,差异有统计学意义(P<0.05);2 Hz电针组、15 Hz电针组比较差异无统计学意义(P>0.05)。结论电针治疗能减轻豚鼠膜迷路积水,其中2/15 Hz电针效果最佳,其作用机制可能与下调血浆AVP的表达有关。Objective To investigate the effect of different frequency electroacupuncture on cochlear morphology and plasma arginine vasopressin(AVP)concentration in guinea pigs with endolymphatic hydrops,select the suitable parameters of electroacupuncture for endolymphatic hydrops and preliminarily explore the possible mechanism of electroacupuncture treatment for Meniere's disease.Method Forty healthy male guinea pigs were randomized to blank group,model and electroacupuncture(3 subgroups according to different frequencies)groups,eight in each group.A model of endolymphatic hydrops was made by intraperitoneal injection of desmopressin acetate in all the groups except the blank group.The blank group was not treated.The model group was fastened by the same method as in the electroacupuncture group after model making.Various electroacupuncture subgroups received electroacupuncture of three different frequencies 2 Hz,15 Hz and 2/15 Hz respectively at points Baihui(GV20)and Tinggong(SI19).The degree of cochlear hydrops was examined,the R value was calculated and plasma AVP concentration was measured after the completion of intervention.Result Cochlear R value increased in the model group of guinea pigs compared with the control group(P<0.05)and decreased in all the electroacupuncture subgroups of guinea pigs compared with the modelgroup(P<0.05).When various electroacupuncture subgroups were compared,cochlear R value decreased in the 2/15 Hz electroacupuncture subgroup than in the 2 Hz and 15 Hz electroacupuncture subgroups with a statistically significant difference(P<0.05)and had no statistically significant difference between the 2 Hz and 15 Hz electroacupuncture subgroups(P>0.05).Plasma AVP concentration increased in the model group of guinea pigs compared with the control group(P<0.05)and decreased in all the electroacupuncture subgroups of guinea pigs compared with the modelgroup(P<0.05).When various electroacupuncture subgroups were compared,plasma AVP concentration decreased in the 2/15 Hz electroacupuncture subgroup th
