硅凝胶假体联合自体脂肪胶(SVF-gel)移植隆乳的疗效观察

摘　　要：目的探讨硅凝胶假体联合自体脂肪胶(SVF-gel)在胸部整形中的临床应用效果方法选取2018年12月至2019年12月于潍坊医学院整形外科医院接受隆乳手术的71例求美者作为研究对象,行自体脂肪干细胞胶(SVF-gel)移植联合硅凝胶假体隆乳术。对手术安全性、术后疗效进行评估,并随访记录6个月后患者乳房美观程度及满意度。术后随访观察6~24个月,总结分析临床疗效和并发症。结果就医者术后并发症如切口感染、脂肪液化、假体移位、包膜挛缩等总发生率为5.6%,术后6月随访统计并发症出现率为4.2%。就医者乳房外观较治疗前有较大程度的改善,满意度大幅提高。结论在隆乳手术中,应用硅凝胶假体联合自体脂肪干细胞胶(SVF-gel)移植隆乳可有效提高就医者乳房的美观程度,降低假体外露、突出于皮肤表面的几率,对比单纯应用自体脂肪颗粒移植的隆乳手术,本手术方式抽取脂肪体积少,二次手术几率小,对比单纯假体植入的隆乳手术降低了假体凸出,假体与胸壁衔接不自然的几率,适应症更广,术后并发症少,值得临床进一步推广实践。Objective To investigate the clinical application of Silicone Gel prosthesis combined with SVF-gel in thoracic plastic surgery Methods Seventy-one patients who had undergone augmentation mammoplasty at the Weifang Medical University from December 2018 to December 2019 were selected for the study.To evaluate the surgical safety of the patients,and to record the aesthetic degree of the patients’breasts after 6 months.After 6~24 months follow-up,the clinical effects and complications were analyzed.Results The total incidence of postoperative complications such as mild infection,fat liquefaction,capsular contracture and prosthesis displacement was 5.6%.After 6 months postoperatively,the number is 4.2%.The patients'breast was obviously more beautiful than before treatment,and the satisfaction degree was greatly improved.Conclusion In breast plastic surgery,the use of Silicone Gel prosthesis combined with SVF-gel implants can effectively improve the beauty of the breasts,reduce the risk of exposure of prosthesis.Comparing to the Breast Augmentation only use autologous fat granule transplantation this method reduced the volume of fat needed and the probability of secondary operation.Comparing to the Breast Augmentation only use Silicone Gel,it reduced the probability of protrusion of the prosthesis and unnatural connection between the prosthesis and the chest wall,the indications are more extensive and the postoperative complications are less,which is worthy of further clinical practice.
