皮下心型缝合在前次瘢痕形成剖宫产术中的应用

摘　　要：目的利用皮下心型缝合对再次剖宫产产妇腹部瘢痕防治中的作用。方法自 2018 年 4 月至2019 年4月,选取我院行瘢痕子宫再次剖宫产产妇共112例,随机分为观察组(56例)和对照组(56例),观察组皮下脂肪采用皮下心型缝合,对照组皮下脂肪采用间断缝合,并根据温哥华瘢痕量表(VSS)进行瘢痕评定(评分越高表示瘢痕越严重)。结果 112 例患者,定期随访 (术后1月、3月、6月、12月)至术后12月,根据温哥华瘢痕量表进行瘢痕评定柔韧性、厚度、血管和色素沉着等方面评估,并进行组间比较,采用皮下心型缝合腹部皮肤瘢痕愈合评分低于对照组,术后1月起至术后12月组间差异均有统计学意义,观察组瘢痕愈合优于对照组。结论利用处理皮下心型缝合对再次剖宫产产妇腹部切口起到减张作用,术后伤口减张好,张力小、瘢痕不明显,患者满意度高,值得在临床进一步推广。Objective To observe the effect of subcutaneous aesthetic suture techniques on the prevention and treatment of abdominal scar in multipara pregnant women who had underwent cesarean section,when they had operation again.Methods There were 112 cases multipara pregnant women who had cesarean section in previous pregnancy in our department from April 2018 to April 2019,were divided into the control group and the observation group according to different suture methods in cesarean section.The subcutaneous aesthetic suture techniques were used on subcutaneous fat layer in observation group(56 casees),the conventional suture were used on those in control group(56 casees).All of them had follow-up to observe scar formation after operation,using the vancouver scar scale(VSS,the higher score represent more serious the scar).Results A follow-up(1month,3months,6months,12months after cesarean section)were proformmed on 112 patients,using vancouver scar scale to assess scar flexibility,thickness,vessels and pigmentation.The scar VSS of two groups were analyzed.The VSS(1 month,m3months,6months,12months after operation)of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion Subcutaneous aesthetic suture techniques can reduce the abdominal scar formation in women underwent twice cesarean section due to little tension and outcome of cosmetic scar after operation,and had highly satisfied on patients.
