不同类型皮瓣用于修复臀骶部深度压疮手术方式选择研究

摘　　要：目的探讨不同类型皮瓣在臀骶部深度压疮手术治疗中的应用效果。方法将84例臀骶部深度压疮患者随机分为A组(穿支皮瓣)、B组(带蒂复合组织真皮瓣)、C组(臀大肌肌皮瓣)各28例,观察两组患者的手术治疗效果。结果 A组、B组、C组患者的术后恢复优良率分别为96.43%、100%以及78.57%,对比差异具有统计学意义(χ~2=9.662,P<0.05),并发症发生率分别为3.57%、3.57%以及21.43%,对比差异具有统计学意义(χ~2=6.908,P<0.05)。结论在臀骶部深度压疮的修复手术治疗期间,穿支皮瓣、带蒂复合组织真皮瓣以及臀大肌肌皮瓣等不同类型皮瓣的应用,均有着良好的治疗效果。在实际的治疗过程中,穿支皮瓣、带蒂复合组织真皮瓣的优势更为显著。Objective To investigate the effect of different types of skin flaps in the treatment of deep pressure sores in the buttocks and sacrum.Methods 84 patients with deep pressure ulcer in the gluteal and sacral region were randomly divided into group A(perforator flap),group B(pedicle composite tissue true flap)and group C(gluteus maximus myocutaneous flap),with 28 cases in each group.Results The excellent and good rates of postoperative recovery in group A,group B and group C were 96.43%,100%and 78.57%,respectively.The difference was statistically significant(χ2=9.662,P<0.05),and the incidence of complications was 3.57%,3.57%and 21.43%,respectively.The difference was statistically significant(χ2=6.908,P<0.05).Conclusion The application of perforator flap,pedicled composite tissue dermal flap and gluteus maximus myocutaneous flap has good therapeutic effect in the treatment of deep gluteal pressure ulcer.In the actual treatment process,perforator flap and pedicled composite tissue dermal flap have more significant advantages.
