摘　　要：目的:探讨单发性肝转移瘤(IHM)的CT及MRI影像特征,以提高诊断及鉴别诊断能力。方法:回顾性收集广西医科大学第一附属医院2010年2月至2018年4月收治的IHM患者69例,65例患者CT平扫加增强检查,37例患者同时CT和MRI平扫加增强检查,41例行MRI平扫加增强检查,其中13例使用特殊对比剂检查。将病灶分为直径≤3 cm组(n=47)和直径>3 cm组(n=22),分析其临床特点及CT和MRI的强化特征等。结果:直径>3 cm组IHM在T2WI表现为不均匀高信号、DWI出现“反靶征”,与直径≤3 cm组IHM比较,差异均具有统计学意义(P<0.05);两组转移瘤的CT、MRI强化方式均有其特征性。13例IHM在肝胆期均表现为低信号,其中8例出现中央或边缘斑片状稍高信号。结论:IHM在CT和MRI表现上有其特征性,有助于诊断和鉴别诊断,可为临床提供重要的影像学信息。Objective:To explore the CT and MRI imaging features of solitary intrahepatic metastasis(IHM)in order to improve diagnosis and differential diagnosis.Methods:Sixty-nine cases of IHM from February 2010 to April 2018 in the First Affiliated Hospital of Guangxi Medical University were retrospectively collected;65 patients had plain and enhanced CT scan,37 patients had plain and enhanced CT and MRI scan simultaneously,and 41 patients had plain and enhanced MRI scan,of which 13 cases were examined using special contrast agents.The lesions were divided into a diameter≤3 cm group(n=47)and a diameter>3 cm group(n=22),and their clinical characteristics and the CT and MRI enhancement characteristics were analyzed.Results:IHM in the diameter>3 cm group showed inhomogeneous high signal in T2WI and“reverse target sign”in DWI,which were significantly different from those in the diameter<3 cm group(P<0.05).The IHM in both groups showed specific CT and MRI enhancement patterns.All of the 13 cases of IHM showed low signal in hepatobiliary phase images,including 8 cases with a central or marginal patchy slightly higher signal.Conclusion:IHM has its specific CT and MRI characteristics,which is helpful for diagnosis and differential diagnosis and can provide important imaging information for clinical practice.
