摘 要:目的探索血浆长链非编码RNA(lncRNA)PITPNA-AS1表达水平与缺血性脑卒中(IS)发病风险的关联,分析PITPNA-AS1是否可作为IS的生物标记物。方法采用病例对照研究,收集深圳地区IS新发病例及正常对照各302例,按同性别、年龄(±3)岁进行1∶1匹配,通过微滴式数字PCR检测血浆PITPNA-AS1的表达水平,采用条件logistic回归分析PITPNA-AS1表达水平与IS发病风险之间的关联,采用受试者工作特征曲线及重新分类的方法分析PITPNA-AS1作为IS生物标记物的可行性。结果与对照组相比,IS病例组血浆PITPNA-AS1的表达水平较高,差异有统计学意义(P<0.001),校正其他危险因素后,OR为2.87(95%CI:1.84~4.46)。与只纳入其他危险因素的模型相比,加入PITPNA-AS1能增加模型的曲线下面积(P=0.013),净重分类改善度为0.35(95%CI:0.19~0.51),综合区分改善度为0.10(95%CI:0.03~0.17)。结论血浆lncRNA PITPNA-AS1表达水平与IS发病风险有关,PITPNA-AS1具有成为IS生物标记物的潜能。Objective The aim of this study was to investigate the association of the expression level of plasma long non-coding RNA(lncRNA)PITPNA-AS1 with the risk of ischemic stroke(IS),and to examine the potential of using plasma PITPNA-AS1 as a biomarker for IS.Methods A total of 302 pairs of newly diagnosed IS patients and controls matched on age(±3 years)and sex were recruited in Shenzhen.Droplet digital polymerase chain reaction was applied to measure the lncRNA expression levels.Conditional logistic regression model was conducted to assess the association between plasma PITPNA-AS1 expression level and IS risk.The receiver operating characteristic(ROC)curve and reclassification analysis were used to assess the potential of PITPNA-AS1 as a novel biomarker for the detection of IS.Results The expression levels of lncRNA PITPNA-AS1 in the IS patients were significantly higher than those in the controls(P<0.001).The higher expression level of PITPNA-AS1 was associated with increased risk of IS,and the adjusted odds ratio was 2.87(95%CI:1.84,4.46).Inclusion of PITPNA-AS1 to the well-recognized risk factors improved the area under the ROC curve(P=0.013),with a net reclassification improvement of 0.35(95%CI:0.19,0.51)and an integrated discrimination improvement of 0.10(95%CI:0.03,0.17).Conclusions The plasma PITPNA-AS1 level was positively associated with the risk of IS.The results suggested that the plasma PITPNA-AS1 might serve as a promising molecular biomarker for IS.
