荆州市艾滋病检测实验室网络建设与运行现状分析

摘　　要：目的了解荆州市艾滋病检测实验室网络建设与运行状况,为完善实验室布局和改善艾滋病防治策略提供参考。方法对批准备案的荆州市147家艾滋病检测实验室的实验室类型、区域分布、覆盖率和检测量等进行统计分析。结果荆州市共有1家确证实验室,42家筛查实验室,104家检测点,其中医疗机构128家(87.1%),疾控机构7家(4.8%),妇幼保健机构8家(5.4%),采供血机构3家(2.0%),其他机构1家(0.7%)。各县、区的实验室数量介于5~31家之间,实验室数量的地理分布密度介于0.49~2.33家/100 km^2,服务每万人的实验室数量介于0.086~0.414之间。全市二级及以上医疗机构艾滋病实验室覆盖率达到93.2%,乡镇卫生院实验室覆盖率达78.6%。2019年医疗机构HIV筛查检测量占全市的71.0%,筛查阳性数占56.3%。结论荆州市的艾滋病检测实验室网络体系建设较为完善,运行良好,布局基本合理,但需要进一步加快乡镇卫生院检测点及医疗机构的确证实验室建设。OBJECTIVE To understand the construction and running status of HIV laboratory network in Jingzhou,and to provide references for optimizing laboratory distribution and mending HIV/AIDS prevention and control strategy. METHODS Statistical analysis was performed on the laboratory types,regional distribution,coverage,and testing volume of the 147 approved HIV laboratories. RESULTS There were 1 HIV confirmatory laboratory,42 screening laboratories and 104 testing sites in Jingzhou,which were distributed in hospitals(128,87.1%),centers for disease control and prevention(7,4.8%),maternal and childcare institutions(8,5.4%),and blood collection and supply institutions(3,2.0%)and other health-care facilities(1,0.7%). The number of laboratories in each county/district ranged from 5 to 31. The geographical distribution density of laboratories ranged from 0.49 and 2.33 per 100 square kilometers. The number of laboratories serving each 10 000 persons ranged from 0.086 to 0.414. The coverage rate of HIV laboratories in hospitals of grade II and III reached to 93.2%(40/43). The coverage rate of HIV laboratories in township health centers was 78.6%(92/117). In 2019,hospitals contributed 71.0% of the cumulative HIV screening tests and 56.3% of the HIV screening positive tests in the whole city. CONCLUSION The HIV laboratory network in Jingzhou was relatively complete,running well,and the distribution was basically reasonable. But the construction of laboratories in township health centers and confirmatory laboratories in medical institutions needed to be further accelerated.
