摘 要:目的探讨乙型肝炎病毒(HBV)母婴传播的相关风险因素,为HBV母婴传播的防治提供参考。方法选取2018年1月—2019年12月在甘肃省妇幼保健院分娩的224例乙肝表面抗原(HBsAg)阳性的产妇及其分娩新生儿224例,观察HBV母婴传播宫内感染结果,调查相关风险因素。结果 46例新生儿脐带血HBV DNA阳性,阳性率为20.54%,6个月后11例婴儿诊断为HBV母婴传播宫内感染,感染率为4.91%;单因素分析产妇年龄、分娩孕周、分娩方式、妊娠合并症或并发症与HBV母婴传播宫内感染无关,HBeAg与HBV DNA定量不同时HBV母婴传播宫内感染率不同,差异有统计学意义。新生儿性别、出生体重、1 min Apgar评分、喂养方式与HBV母婴传播宫内感染无关;非条件多因素logistic回归分析显示产妇HBeAg阳性、产妇HBV DNA定量>106500copies/mL和脐带血HBV DNA定量>106500copies/mL是HBV母婴传播宫内感染的相关风险因素。结论产妇HBeAg阳性及HBV DNA定量高水平、脐带血HBV DNA定量高水平是HBV母婴传播的高风险因素,产妇年龄、分娩孕周、分娩方式、妊娠合并症或并发症、新生儿性别、出生体重、1 min Apgar评分和喂养方式作为风险因素影响HBV母婴传播的证据不足。OBJECTIVE To investigate risk factors related to mother-to-child transmission of hepatitis B virus(HBV),it provides refer⁃ences for the prevention and treatment of mother-to-child transmission of HBV.METHODS 224 cases pregnant women and 224 cases their newborns with HBsAg positive were selected from January 2018 to December 2019,to observe the results of intrauterine HBV transmission and investigate the related risk factors.RESULTS 46 cases neonates had HBV DNA positive in umbilical cord blood,the positive rate was 20.54%,the 11 cases infants were diagnosed with intrauterine HBV transmission after 6 months,the infection rate was 4.91%.Univariate analysis,Maternal age,gestational age,mode of delivery,pregnancy complications or complications were not correlated with HBV intrauterine infection.HBeAg and HBV DNA quantification were different,and the intrauterine infection rate of HBV mother-to-child transmission was different,the difference was statistically significant.Neonatal gender,birth weight,Apgar score of 1min and feeding pattern were not correlated with intrauterine infection transmitted by HBV.Unconditioned multivariate Lo⁃gistic regression analysis showed that maternal HBeAg positive,maternal HBV DNA quantitative>106500 copies/ml,and umbilical cord blood HBV DNA quantitative>106500 copies/ml were related risk factors of intrauterine infection with mother-to-child trans⁃mission of HBV.CONCLUSION Maternal HBeAg positive and HBV DNA quantitative high level and umbilical cord blood HBV DNA quantitative level are high risk factors for HBV mother-to-child transmission,there is insufficient evidence that maternal age,gesta⁃tional weeks,mode of delivery,pregnancy complications or complications,neonatal sex,birth weight,1min Apgar score and feeding pattern were risk factors affecting mother-to-child transmission of HBV.
