摘  要:目的探讨胰激肽原酶对大鼠急性心肌梗死内皮祖细胞血管生成能力的影响。方法将造模成功的急性心肌梗死大鼠42只随机分为三组:模型组、实验1组与实验2组,每组各14只。分别给予蒸馏水、阿司匹林30 mg/(kg·d)、胰激肽原酶800 U/(kg·d)治疗,持续给药8周,超声测量各组大鼠心脏左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)、左室收缩压(LVESP),流式细胞仪检测各组大鼠CD34+细胞和CD133+细胞表达及West-Blot法检测大鼠心肌细胞结缔组织生长因子(CTGF)、血管内皮生长因子(VEGF)的表达水平。结果给药后4周与8周,实验1组、实验2组的LVEDD与LVESD都低于模型组,实验2组也低于实验1组,对比差异都有统计学意义(P<0.05)。给药后8周,实验1组、实验2组的LVESP、CD34+细胞和CD133+细胞比例都高于模型组,实验2组也高于实验1组,对比差异都有统计学意义(P<0.05)。模型组心肌间质胶原纤维明显增生,心肌细胞肿胀,实验1组心肌细胞间胶原纤维减少,非梗死区心肌细胞肿胀;实验2组心肌细胞呈现正常组织结构,呈条索状与心肌纤维平行排列。给药后8周,实验1组、实验2组心肌细胞内CTGF、VEGF相对表达水平高于模型组,实验2组也高于实验1组,对比差异都有统计学意义(P<0.05)。结论胰激肽原酶在大鼠急性心肌梗死中的应用能在一定程度上增加内皮祖细胞(EPCs)数量,改善心功能与血流动力学状况,激活CTGF、VEGF的表达,从而有利于血管生成。Objective To investigate the effects of pancreatic kininogenase on the angiogenesis of endothelial progenitor cells in rats with acute myocardial infarction.Methods 442 rats with acute myocardial infarction were randomly divided into three groups:model group,experimental group 1 and experimental group 2,with 14 rats in each group.They were treated with distilled water,aspirin 30 mg/(kg·d),and pancreatic kininogenase 800 U/(kg·d)for 8 weeks.Ultrasound was used to measure the left entricular end-diastolic diameter of the heart of each group of rats.diameter,LVEDD),left entricular end-systolic diameter(LVESD),left ventricular end-systolic pressure(LVESP).Flow cytometry was used to detect the expression of CD34+cells and CD133+cells in each group,and the West-Blot method was used to detect the expression levels of connective tissue growth factor(CTGF)and vascular endothelial growth factor(VEGF)in rat cardiomyocytes.Results At 4 weeks and 8 weeks after the administration,the LVEDD and LVESD of experiment 1 and experiment 2 were lower than the model group,and experiment 2 was also lower than experiment 1,and the difference was statistically significant(P<0.05).8 weeks after the administration,the proportions of LVESP,CD34+cells and CD133+cells in the experimental group 1 and the experimental group 2 were higher than those in the model group,and the experimental group 2 was also higher than that in the experimental group 1,and the difference was statistically significant(P<0.05).The collagen fibers in the myocardial interstitium of the model group were significantly proliferated and the myocardial cells were swollen.The collagen fibers in the myocardial cells of the experimental group were decreased,and the myocardial cells in the non-infarct area were swollen.The cardiomyocytes of the experiment 2 group showed normal tissue structure,arranged in a cord shape parallel to the myocardial fibers.8 weeks after the administration,the relative expression levels of CTGF and VEGF in the cardiomyocytes of the experimental grou
