摘 要:目的观察静脉应用阿替洛尔控制心率对Double-Flash模式在CT冠状动脉造影(CTCA)临床应用率中的影响。方法对心率≤100次/min的266例受检者进行CTCA检查。将受检者随机分为两组,A组为自然心率组;B组为心率控制组,对其中心率>65次/min者静推阿替洛尔注射液2.5 mg。CTCA中对心率≤65次/min者应用Double-flash模式扫描,对>65次/min者应用回顾性心电门控模式扫描。分别记录A、B两组中两种扫描模式的使用例数、图像质量、辐射剂量及B组中药物使用的不良反应发生率。结果Double-flash扫描模式A组应用率为63.09%(94/149),B组为97.44%(114/117),差异有统计学意义(P<0.05)。A组有效辐射剂量(ED)和CT容积剂量指数(CTDIvol)分别为(4.14±0.55)msv和(14.67±0.81)mGy,B组分别为(1.51±0.87)msv和(3.13±0.78)mGy,差异均有统计学意义(P<0.05)。B组中无药物使用的不良反应者。A组97.98%(147/149)、B组97.44%(114/117)图像可诊断,差异无统计学意义(P>0.05)。结论静脉应用阿替洛尔可安全有效地提高CTCA中Double-Flash扫描模式的应用率。Objective To observe the effect of intravenous application of atenolol to control heart rate on the clinical application rate of Double-Flash mode in CT coronary angiography(CTCA).Methods CTCA examination was performed on 266 subjects with heart rate≤100 bpm.The subjects were randomly divided into two groups,group A was the natural heart rate group;group B was the heart rate control group,and those who had a central rate of>65 bpm were given atenolol injection 2.5 mg.In CTCA,double-flash mode scan is applied to those with heart rate≤65 bpm,and retrospective ECG gated mode scan is applied to those with>65 bpm.The number of use cases,image quality,radiation dose,and incidence of adverse reactions of drug use in group B were recorded in groups A and B,respectively.Results The application rate of group A in Double-flash scanning mode was 63.09%(94/149),group B was 97.44%(114/117),the difference was statistically significant(P<0.05).The effective radiation dose(ED)and CT volume dose index(CTDIvol)of group A were(4.14±0.55)msv and(14.67±0.81)mGy,and group B were(1.51±0.87)msv and(3.13±0.78)mGy,The differences were statistically significant(P<0.05).There were no adverse reactions of drug use in group B.97.98%(147/149)of group A and 97.44%(114/117)of group B can be diagnosed,the difference is not statistically significant(P>0.05).Conclusions Intravenous administration of atenolol may increase the application rate of Double-Flash mode in CTCA effectively and safely.
