摘 要:目的探讨不同体位颈性前庭诱发肌源性电位(cervical VEMP,cVEMP)的稳定记录方法.方法对30名正常青年志愿者分别采用仰卧抬头和坐位转颈两种不同体位,进行气导短纯音刺激诱发的cVEMP检测,记录和对比两种体位下的引出率和参数特征.结果两种体位cVEMP的引出率均为100%,P13、N23波的潜伏期及P1-N1的波间期,耳间振幅比均无统计学意义(P>0.05);左、右耳均在100 dB声刺激下,仰卧抬头和坐位转颈两种体位的P1和N1波潜伏期及P1-N1波间期,差异均无统计学意义.两种体位下,100 dB nHL声刺激所引起的P13-N23波振幅比均显著高于90 dB nHL,同耳P1-N1波振幅比在仰卧抬头位高于坐位转颈位,差异均有统计学意义.不同体位下P1-1的耳间差比无明显差异,P1-N1振幅不对称率比较,差异有统计学意义.坐位转颈位的振幅不对称率显著高于仰卧抬头位.结论仰卧抬头和坐位转颈两种体位均是cVEMP测试适合的检测体位,仰卧抬头位可获得更高振幅,易于分辨.OBJECTIVE To explore the stable recording method of cervical vestibular evoked myogenic potential(VEMP) in different postures.METHODScVEMP induced by toneburst stimulation was detected in 30 normal young volunteers by two different postures:supine head up and sitting neck rotation.The elicitation rate and parameter characteristics of the two postures were recorded and compared.RESULTS The elicitation rates of cVEMP in the two postures were both 100%.There was no statistical significance(P>0.05) in the latency of P1 and N1 waves,the interval of P1-N1 waves and the amplitude ratio.The amplitude ratio of P13-N23 wave induced by 100 dB nHL stimulation was significantly higher than that of 90 dB in two postures.The amplitude ratio of P1-N1 in supine head up position was higher than that in sitting and turning neck position,and the differences were statistically significant.There was no significant difference in the ratio of P1-N1 in different postures,but there was significant difference in the amplitude asymmetry of P1-N1.The amplitude asymmetry of sitting position to neck position was significantly higher than that of supine head up position.CONCLUSIONLying position and sitting position are both suitable for cVEMP test in young people.Lying position can obtain higher amplitude and easy to recognize.
