腋静脉留置针技术在新生儿重症监护室早产儿中的应用

摘　　要：目的探讨腋静脉留置针技术在新生儿重症监护室(NICU)早产儿中的应用。方法选取2019年6~12月我院收治的110例NICU早产儿为研究对象,采用随机数字表法将其分为研究组和对照组,每组各55例,研究组患儿采用腋下静脉留置套管针输液,对照组患儿行采用静脉留置四肢和头皮输液,比较两组患儿的置针效果、留置时间及相关并发症发生情况。结果研究组患儿首针穿刺成功率高于对照组,穿刺时间短于对照组,非计划拔管率低于对照组,差异有统计学意义(P<0.05);两组患儿留置针意外脱落率比较,差异无统计学意义(P>0.05);研究组患儿留置针留置时间>72 h占比高于对照组,留置时间长于对照组,差异均有统计学意义(P<0.05);研究组患儿并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论腋静脉留置针技术可有效提升NICU新生儿的置针效果,降低并发症发生率,值得临床进一步推广应用。Objective To explore the application of axillary vein indwelling needle technique in premature infant of neonatal intensive care unit(NICU).Methods A total of 110 cases of NICU premature infants admitted to our hospital from June to December 2019 were selected as the research objects,they were divided into study group and control group using random number table method,55 cases in each group.The study group received intravenous indwelling trocar infusion,and the control group received intravenous infusion of extremities and scalp.The effect of needle placement,indwelling time and the incidence of related complications were compared between the two groups.Results The success rate of first needle puncture in the study group was higher than that in the control group,the puncture time was shorter than that in the control group,the unplanned extubation rate was lower than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the accidental drop-off rate of the indwelling needle between the two groups(P>0.05).The proportion of indwelling needle retention time>72 h in the study group was higher than that in the control group,and the indwelling time was longer than that in the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion The technique of axillary vein indwelling needle can effectively improve the effect of NICU neonates and reduce the incidence of complications,it is worthy of further clinical application.
