摘 要:目的对山东枣庄地区0~12岁儿童人巨细胞病毒(HCMV)感染情况进行流行病学调查,进而为枣庄地区儿童HCMV感染的防控提供依据。方法收集2015年1月至2016年12月枣庄市妇幼保健院0~12岁2 750例儿童血清样本,其中男1 656例,女1 094例。2 750例儿童按照年龄分为0~<28天组、28天~<6月龄组、6月龄~<1岁组、1~<3岁组、3~<6岁组及6~12岁组。回顾性分析不同年龄组HCMV IgM抗体和IgG抗体阳性率、分布及临床特点。结果在2 750例儿童中,HCMV血清学阳性率为71.89%,总HCMV IgM、总IgG抗体阳性率分别为7.20%、68.69%。0~<28天组总HCMV IgM抗体阳性率为0.30%,与28天~<6月龄、6月龄~<1岁、1~<3岁、3~<6岁及6~12岁组比较,差异均有统计学意义(χ^2值分别为300.36、316.35、338.69、170.51、63.69,均P<0.05)。原发感染率在0~<28天组最低(0.30%),随着年龄增长逐渐升高,在1~<3岁组达到最高,为13.53%,3岁以后下降,差异有统计学意义(χ^2=179.20,P<0.05)。HCMV IgM抗体阳性率在不同季节间比较差异无统计学意义(χ^2=2.29,P>0.05),HCMV IgG抗体阳性率在冬季最低(44.44%),与春、夏、秋三个季节比较差异均有统计学意义(χ^2值分别为149.66、129.48、113.35,均P<0.05)。在198例HCMV IgM抗体阳性儿童中,84例儿童诊断为呼吸系统疾病,其中以肺炎最多(21.21%)。结论枣庄地区0~12岁儿童HCMV感染普遍,尤其是呼吸系统疾病,临床上应重视对儿童HCMV感染的防控。Objective To investigate infection status,distribution and clinical characteristics of human cytomegalovirus(HCMV) infection in children aged 0~12 years in Zaozhuang region so as to provide basis for prevention and treatment of the disease.Methods The serum samples of 2 750 children aged 0 to 12 years who visited to Zaozhuang Municipal Maternity and Child Health Hospital from January 2015 to December 2016,including 1 656 boys and 1 094 girls were taken.The children were divided into six age groups according to their age:0 day~<28 days age group,28 days~<6 months age group,6 months~<1 year age group,1 year~<3 years age group,3 years~<6 years age group and 6 years^12 years age group.Anti-HCMV IgM and IgG antibodies were quantitatively detected in magnetism particulate immunochemistry luminescence(MPICL) method.Anti-HCMV IgM and IgG antibodies positive detection rates,distribution and clinical characteristics in the children in different age groups were retrospectively analyzed.Results In 2 750 serum samples,the serological positive rate of HCMV was 71.89%,the overall positive rates of anti-HCMV IgM and IgG antibodies were 7.20% and 68.69% respectively.The overall positive rate of anti-HCMV IgM antibody in the neonates in 0 day~<28 days age group was 0.30%,there was significant difference as compared with that in other five age groups(χ^2=300.36,316.35,338.69,170.51 and 63.69 respectively,all P<0.05).The primary infection rate of HCMV was lowest in the 0 day~<28 days age group(0.30%),with increase in age the primary infection rate of HCMV raised gradually,at 3 years old,it reached to the highest(13.53%),and then declined(χ^2=179.20,P<0.05).There was no significant difference in the seroprevalence of anti-HCMV IgG antibody among the four seasons(χ^2=2.29,P>0.05),The seroprevalence of anti-HCMV IgG antibody was the lowest in winter(44.44%) and the differences was statistically significant as compared with that in other three seasons(χ^2=149.66,129.48 and 113.35 respectively,all P<0.05).Among 198 children with posi
