摘　　要：目的检测骨髓增生异常综合征(MDS)患者血清长链非编码RNA HOXB-AS3(LncRNA HOXB-AS3)水平,并探讨其与MDS的关系。方法选取2017年3月~2019年12月于本院门诊就诊或住院治疗的74例MDS患者作为MDS组,再将74例MDS患者按照国际预后积分系统(IPSS)重新分为低危组、中危Ⅰ组、中危Ⅱ组和高危组,同时按照WHO分级重新分为难治性贫血型(RA)组、难治性血细胞减少伴多系发育异常型(RCMD)组、难治性贫血伴原始细胞增多-Ⅰ型(RAEB-Ⅰ)组和难治性贫血伴原始细胞增多-Ⅱ型(RAEB-Ⅱ)组;同时选取同期68例健康体检人群作为对照组。采用实时荧光定量PCR(qRT-PCR)法检测血清中LncRNA HOXB-AS3水平;分析血清HOXB-AS3 mRNA表达与IPSS评分的关系;分析血清HOXB-AS3 mRNA表达与WHO分级的关系;分析血清中HOXB-AS3 mRNA水平对MDS的诊断价值。结果MDS组血清中HOXB-AS3 mRNA表达水平显著高于对照组(P<0.05);高危组血清中HOXB-AS3 mRNA表达水平显著高于低危组、中危Ⅰ组和中危Ⅱ组(P<0.05);RAEB-Ⅱ组血清中HOXB-AS3 mRNA表达水平显著高于RA组和RCMD组(P<0.05);HOXB-AS3 mRNA表达水平与IPSS评分、WHO分级均呈正相关(r=0.550、0.597,P均<0.05)。结论血清LncRNA HOXB-AS3表达可能对MDS的发生具有预测作用,对评估病情发展状况有重要意义,可为MDS的早期干预提供一定参考依据。Objective To detect the level of serum long non-coding RNA HOXB-AS3(LncRNA HOXB-AS3)in patients with myelodysplastic syndrome(MDS)and to explore its relationship with MDS.Methods From March 2017 to December 2019,a total of 74 MDS patients in our hospital were treated as MDS group.The 74 MDS patients were divided into low-risk group,middle-risk groupⅠ,middle-risk groupⅡand high-risk group according to the international prognostic score system(IPSS).According to WHO classification,the patients were divided into refractory anemia(RA)group,refractory cytopenia multiple dysplasia(RCMD)group,refractory anemia with excess blasts typeⅠ(RAEB-Ⅰ)group and refractory anemia with excess blasts typeⅡ(RAEB-Ⅱ)group.68 healthy people were selected as the control group.The level of serum LncRNA HOXB-AS3 was detected by real-time fluorescent quantitative PCR(qRT-PCR).The relationship between the expression of HOXB-AS3 mRNA and IPSS score was analyzed.The relationship between the expression of HOXB-AS3 mRNA in serum and WHO grading was analyzed.The diagnostic value of HOXB-AS3 mRNA level in serum for MDS was analyzed.Results The expression level of HOXB-AS3 mRNA in MDS group was significantly higher than that in control group(P<0.05).The expression level of serum HOXB-AS3 mRNA in high risk group was significantly higher than that in low risk group,medium risk groupⅠand medium risk groupⅡ(P<0.05).The expression level of HOXB-AS3 mRNA in RAEB-Ⅱgroup was significantly higher than that in RA group and RCMD group(P<0.05).The expression of HOXB-AS3 mRNA was positively correlated with IPSS score and WHO grade(r=0.550,0.597,P<0.05).Conclusion The expression of serum LncRNA HOXB-AS3 may have a predictive effect on the occurrence of MDS and may be of great significance for the evaluation of the development of MDS,and provide some reference for the early intervention of MDS.
