摘　　要：目的探讨肩部推拿手法结合在肌骨超声引导下关节腔注射对早期原发性冻结肩患者的疼痛程度及关节活动度的影响。方法将诊断为早期原发性冻结肩的患者44例,随机分为试验组和对照组,每组各22例。试验组采用推拿结合超声引导关节腔内注射治疗;对照组采用超声引导关节腔注射治疗,治疗1周后,2周后观察VAS评分、Melle评分。结果治疗1周、2周后,2组的VAS评分均较治疗前降低,但治疗1周后,2周后试验组和对照组的VAS评分组间比较时差异均无统计学意义(P>0.05)。治疗1周、2周后,2组的Melle评分均较治疗前明显降低(P<0.05),但试验组治疗1周后、2周后的Melle评分低于对照组,且差异具有统计学意义(P<0.05)。结论推拿结合超声引导关节腔注射治疗早期原发性冻结肩疗效显著,且在改善肩关节活动度上优于单纯超声引导关节腔注射。Objective To explore the clinical efficacy of Tuina combined with ultrasound-guided intra-articular injection for the treatment of early primary frozen shoulder.Methods 44 patients with early primary frozen shoulders were enrolled and randomly assigned to group A(experimental)and group B(control).The group A was treated with Tuina combined with ultrasound-guided intraarticular injection.The group B was treated with ultrasoundguided intra-articular injection.1 week and 2 weeks after treatment,the VAS score and Melle score were evaluated and compared.Results After 1 week and 2 weeks of treatment,the VAS scores of the two groups were both significantly lower than baseline(P<0.05),but the difference between the two groups was not statistical.After 1 week and 2 weeks of treatment,the Melle scores of the two groups were both lower than baseline(P<0.05),but the Melle scores of the group A were lower than the control group(P<0.05).Conclusion Tuina combined with ultrasound-guided intraarticular injection for the treatment of early primary frozen shoulder is effective,and it is better than simple ultrasoundguided intra-articular injection in improving shoulder mobility.It is worthy of clinical application.
