摘　　要：目的:探讨CT值判断肾积液是否合并感染的临床价值和意义。方法:分析2018年1月1日至2020年5月31日我院泌尿外科收治的321例上尿路结石合并肾积液患者的病历资料。男155例,女166例,年龄16~90岁,平均年龄(51.3±14.2)岁。其中无感染肾积液135例,普通感染肾积液103例,肾积脓患者83例。患者术前均常规行泌尿系统CT检查,比较无感染肾积液、普通感染肾积液、肾积脓3组患者肾积液CT值的差异。结果:无感染肾积液组CT值(4.86±3.23)HU,95%CI(3.91-5.72)HU;普通感染肾积液组CT值(9.23±5.86)HU,95%CI(7.31-10.22)HU;肾积脓组CT值(16.49±6.36)HU,95%CI(15.35-18.43)HU。3个组之间的肾积液CT值比较差异有统计学意义(P<0.01)。结论:肾积液CT值在一定程度上可以评估肾积液的感染情况,术前通过CT值判断肾积液是否合并感染具有一定的临床意义。Objective:To explore the clinical value and significance of CT value in identifying whether hydronephrosis is accompanied with infection.Methods:The medical records of 321 patients(155 males and 166 females,aged 16-90 years,with an average age of 51.3±14.2 years)with upper urinary tract calculi and hydronephrosis who were treated in department of urology from January 1,2018 to May 31,2020 were analyzed.135 cases had hydronephrosis with no infection,103 cases with common infection and 83 cases with pyonephrosis.All patients underwent routine CT examination of urinary system before operation,and the differences of CT values of hydronephrosis among non-infected hydronephrosis group,common infected hydronephrosis group and pyonephrosis group were compared.Results:The CT value of the non-infected hydronephrosis group was 4.86±3.23 HU,95%CI(3.91-5.72)HU,while the CT value of the common infected hydronephrosis group was 9.23±5.86 HU,95%CI(7.31-10.22)HU.And the CT value of the pyonephrosis group was 16.49±6.36 HU,95%CI(15.35-18.43)HU.There was a significant difference in CT values of hydronephrosis among three groups(P<0.01).Conclusion:CT value of hydronephrosis is helpful to evaluate the infection of hydronephrosis.And it has certain clinical value to predict infection of hydronephrosis by CT value before operation.
