摘　　要：目的观察沙库巴曲缬沙坦钠片在射血分数降低的心力衰竭(HFrEF)老年患者中的临床及其对半乳糖凝集素3(Gal-3)的干预作用。方法将符合入选标准的60例患者分为A组(常规治疗加沙库巴曲缬沙坦钠片)和B组(常规治疗加贝那普利),每组30例。治疗并随访6个月,比较2组患者治疗前后明尼苏达心衰生活质量量表(MLHFQ)评分、血清Gal-3和N末端B型利钠肽原(NT-proBNP)浓度、超声心动图指标的变化。结果治疗前2组患者比较,MLHFQ评分、血清Gal-3和NT-proBNP浓度、左心房内径(LA)、左心室舒张末期内径(LVEDD)、左心室后壁厚度(LVPWT)、左心室射血分数(LVEF)均无统计学差异(P>0.05)。与治疗前比较,2组患者治疗后MLHFQ评分、血清NT-proBNP和Gal-3浓度下降,LA变小,LVEF提高(P<0.05)。治疗后A组MLHFQ评分、血清NT-proBNP和Gal-3浓度、LA低于B组,LVEF高于B组(P<0.05)。结论沙库巴曲缬沙坦钠片治疗HFrEF老年患者可提高生活质量和预后,降低Gal-3水平,有效改善心脏结构。Objective To observe the clinical application of sacubitril valsartan sodium tablet and its effect on galectin 3(Gal-3)in elderly patients with heart failure and reduced ejection fraction(HFrEF).Methods Sixty patients who met the inclusion criteria were divided into group A(regular anti-heart failure treatment plus sacubitril valsartan sodium tablet)and group B(regular anti-heart failure treatment plus benazepril)(n=30 for each group).Patients were treated and followed up for 6 months.Minnesota Living with Heart Failure Questionnaire(MLHFQ)score,serum Gal-3 and N-terminal pro-brain natriuretic peptide(NT-proBNP)concentrations,and indicators of echocardiography were compared between the two groups before and after treatment.Results Before treatment,there was no statistically significant intergroup difference in MLHFQ score,serum NT-proBNP and Gal-3 concentrations,left atrial diameter(LA),left ventricular end diastolic diameter(LVEDD),left ventricular posterior wall thickness(LVPWT),or left ventricular ejection fraction(LVEF)(P>0.05).MLHFQ score,serum NT-proBNP and Gal-3 concentrations,and LA decreased,and LVEF increased in both groups after treatment(P<0.05).After treatment,MLHFQ score,serum NT-proBNP and Gal-3 concentrations,and LA were lower in group A than in group B,while LVEF was higher in group A than in group B(P<0.05).Conclusion The treatment of elderly HFrEF patients with sacubitril valsartan sodium tablet can improve the quality of life and prognosis,reduce serum Gal-3 level,and effectively improve heart structure.
