农村基本公共卫生服务效率影响因素空间分析

摘　　要：目的:为实现公共卫生资源配置均等化的目标,科学考察农村基本公共卫生服务效率,为提升农村基本公共卫生服务效率建言献策。方法:利用DEA-Malmquist指数测算2011至2017年我国29个省级行政单位的农村基本公共卫生服务效率,再采用空间杜宾模型分析农村基本公共卫生服务效率的影响因素。结果:财政收入分权对农村基本公共卫生服务效率有正向显著影响,而财政支出分权对其有负向显著影响,城镇化水平对农村基本公共卫生服务效率的影响呈现正向的“空间溢出”现象。结论:地方政府应当重视东中西部地域发展与经济发展的差异,调节财政收入分权与支出分权之间的不平衡,加强城镇与乡村之间医疗合作,以提升农村基本公共卫生服务效率。Objective To achieve the objective of equal allocation of public health resources,investigate the efficiency of rural basic public health services,and put forward suggestions for improving it. Methods Based on DEA-Malmquist index,the efficiency of rural basic public health service of 29 provincial administrative units in China from 2011 to 2017 was measured. And its influencing factors were analyzed with spatial Durbin model. Results Decentralization of fiscal revenue has significantly positive influence on the efficiency of rural basic public health services,while decentralization of fiscal expenditure has a significantly negative influence on it. Urbanization has a positive "spatial spillover"effect on the efficiency of basic public health services in rural areas. Conclusion It is suggested for the local governments to attach importance to the differences in regional development and economic development,adjust the imbalance between fiscal revenue decentralization and expenditure decentralization,strengthen medical cooperation between towns and villages,which can improve the efficiency of rural basic public health services.
