摘 要:目的:评价电针对肢体缺血再灌注诱发兔肺损伤时肺组织内质网应激的影响。方法:40只健康雄性新西兰大白兔,采用随机数字表法分为4组:假手术组、模型组、电针组和非穴位电针组,每组10只。采用夹闭股动脉3 h、再灌注4 h的方法制备兔肢体缺血再灌注损伤模型。电针组于模型制备前4 d(每天早8点,30 min/次,1次/d)及模型制备过程中电针肺俞穴和足三里穴(疏密波,刺激电流l^2 mA,频率2/15 Hz,波宽0.2~0.6 ms),以兔出现轻微肌颤为宜;非穴位电针组以相同的参数电针刺激肺俞穴和足三里穴旁开0.5 cm非经非穴处。再灌注4 h时处死兔,取肺组织行HE染色和肺损伤评分,计算肺湿重/干重(W/D)比值;采用TUNEL法确定肺泡上皮细胞凋亡指数(AI);采用Western blotting法检测肺组织葡萄糖调节蛋白78(GRP78)、CCAAT/增强子结合蛋白同源蛋白(CHOP)及caspase-12表达水平。结果:与假手术组比较,其余3组的肺损伤评分、W/D比值及肺泡上皮细胞AI均升高,肺组织GRP78、CHOP及caspase-12表达均上调,差异有统计学意义(P<0.05);与模型组比较,电针组肺损伤评分、W/D比值及肺泡上皮细胞AI降低,肺组织GRP78、CHOP及caspase-12表达下调,差异有统计学意义(P<0.05)。结论:电针刺激肺俞穴和足三里穴减轻兔肢体缺血再灌注诱发肺损伤的机制可能与抑制内质网应激诱导、减轻肺泡上皮细胞凋亡有关。Objective To evaluate the effect of electrical therapy on endoplasmic reticulum stress in rabbit lung injury induced by limb ischemia-reperfusion.Methods Forty New Zealand white rabbits were randomly divided into 4 groups(n=10 each):sham operation group,model group,electrical acupoint stimulation group and non-acupoint stimulation group.Limb ischemia-reperfusion was induced by clamping bilateral femoral arteries for 3 h followed by 4 h.Bilateral electroacupuncture stimulation of Zusanl and Feishu acupoints was performed(frequency 2/5 Hz,disperse-dense wave,intensity1-2 mA,wave length 0.2-0.6 ms)at eight o'clock every morning for 30 min once a day for 4 days before establishment of the model and during the course of establishment of the model in electrical acupoint stimulation group.At the same parameters,the non-acupoint electroacupuncture group opened 0.5 cm non-acupoint by electroacupuncture at FeiShu point and Zusanli point.At 4 h of reperfusion,the rabbits were sacrificed.The lungs were removed microscopic examination and for determination of lung injury scores and wet to dry weight and expression of glucose-regulated protein 78(GRP78)/and CCAAT/enhancer-binding protein homologous protein(CHOP)and caspase-12 in lung tissues(by Western blotting).Results Compared with non-acupoint stimulation group,the lung injury score,W/D ratio and alveolar epithelial cell AI were increased and the GRP78、CHOP and caspase-12 expression of lung tissue were up-regulated in the other three groups(P<0.05),and there was no significant difference in the above indexes in the non-acupoint stimulation group(P>0.05).Conclusion The mechanism of electroacupuncture at Feishu and Zusanli points may be related to the inhibition of endoplasmic reticulum stress-induced lung injury and the reduction of alveolar epithelial cell apoptosis.
