补气活血中药对腰椎间盘突出症术后马尾神经综合征的临床疗效

摘　　要：目的:比较不同黄芪剂量治疗腰椎间盘突出症减压手术后遗留马尾神经综合征(CES)的临床效果,以期得出中药治疗CES的最优方案。方法:选取60例腰椎间盘突出症术后合并CES的患者,随机分治疗组、强化组、对照组,每组20例。应用补气活血中药制剂治疗,对照组行单纯减压手术治疗和传统护理;治疗组在行减压手术治疗后加用补气活血类中药治疗;强化组病例则在减压手术治疗基础上,补气活血中药汤剂中黄芪用量加倍。比较三组的ODI功能评分及其总有效率的差异。结果:减压手术90天后,三组患者的ODI功能评分差异有统计学意义(P<0.05);治疗组和强化组的ODI评分均低于对照组,强化组低于治疗组,差异均有统计学意义。强化组的总有效率为95.0%,高于治疗组(80.0%)及对照组(60.0%),差异有统计意义(P<0.05)。结论:补气活血中药增大黄芪剂量对腰椎间盘突出症术后CES患者病情恢复具有良好的临床疗效,可在临床中推广应用。Objective To compare the efficacy of astragalus in the treatment of cauda equina syndrome(CES)after lumbar disc herniationde compression operation and to find out the best scheme of Chinese medicine for CES.Methods 60 cases of lumbar disc herniation with cauda equina syndrome were selected according to the order of treatment.The patients were divided into treatment group,reinforcement group and control group according to the principle of equal probability.Each group was divided into 20 patients.The patients in the treatment group were treated with traditional Chinese medicine of invigorating Qi and activating blood circulation,which the differential treatment method was used.The control group was treated with the simple decompression operation and traditional nursing.The treatment group was treated with traditional Chinese medicine of invigorating Qi and activating blood circulation after the decompression operation.The cases in the strengthening group were treated with traditional Chinese medicine of invigorating Qi and activating blood circulation.On the basis of the decompression operation,the dosage of astragalus in the decoction of invigorating Qi and activating blood circulation doubled.To compare the efficacy of astragalus in the treatment of CES patients without dosage.Results After 90 days of decompression operation,ODI scores of three groups were compared and the data of three groups were statistically significant(P<0.05)after one-way ANOVA.90 days after decompression,the total effective rate of the three groups(%):95.0 in the strengthening group,80.0 in the treatment group and 60.0 in the control group.The data showed significant difference by the rank sum test(P<0.05).Conclusion The Chinese medicine of invigorating Qi and promoting blood circulation can increase the dosage of astragalus membranaceus,which has a good clinical effect on the recovery of CES patients after operation of lumbar disc herniation,and it can be further applied in clinic.
