摘  要:目的探讨开展品管圈活动(quality control cycle,QCC)在新生儿筛查标本持续质量改进中的应用效果。方法2018年1月至2018年6月开展品管圈活动前的新生儿筛查标本10550份为常规组,2018年7月至2018年12月开展品管圈后的新生儿筛查标本12093份为品管圈组。通过品管圈活动,找出影响新生儿筛查标本质量的原因,确定要因,制定分阶段实施对策与整改措施,并进行效果评估,以期降低新生儿筛查次等标本率。观察并记录两组新生儿筛查标本质量指标,包括血斑部分渗透不全、血斑直径小、血斑重复滴血、血斑出现溶血环等。结果通过品管圈活动,次等血片占合格血片的比重由常规组的9.65%降至品管圈组的1.41%,品管圈组新生儿筛查标本不合格率低于常规组,差异有统计学意义(经Pearson卡方检验,χ^2=768.040,P<0.001)。结论品管圈活动在新生儿筛查标本质量监控中,可以有效降低新生儿筛查次等血片比例,避免了影响实验室检测结果的不良因素,在改善质量与技术方面有重要的意义。Objective To discuss the application effect of quality control circle(QCC)in continuous quality improvement of newborn disease screening samples(NDSS).Methods A total of 10550 NDSS collected before QCC activity carried out during January 2018 and June 2018 were taken as the conventional group,and 12093 samples collected after QCC activity carried out during July 2018 and December 2018 were taken as QCC group.Factors that influenced the quality of newborn disease screening samples were found out through QCC activity,and the essential factors were confirmed.Then rectification measures and strategies implemented in stages were formulated.Finally,the outcomes were evaluated in order to improve the quality of NDSS.Quality factors of NDSS in the two groups including incomplete penetration of blood spot,small diameter,dripping blood repeatedly,and hemolysis etc.were recorded.Results The proportion of inferior-quality samples in the qualified samples decreased from 9.65%in the conventional group to 1.41%in the QCC group.The unqualified rate of neonatal screening samples in the QCC group was lower than that in the conventional group,with statistically significant difference(according to Pearson chi-square test,χ^2=768.040,P<0.001).Conclusion QCC activity can improve the quality of NDSS effectively and avoid adverse factors that influence laboratory testing results,which is significant in enhancement of quality and technique.
